چکیده اصلی

This study aimed to compare maternal and umbilical cord blood pro-BNP levels in pregnant women with a history of COVID-19 infection and to evaluate possible cardiac effects during pregnancy. In this prospective case-control study conducted between January and October 2023, 88 pregnant women were included: 44 with a history of COVID-19 infection after the second trimester and 44 healthy controls. Women with diabetes, hypertension, heart disease, or severe pregnancy complications were excluded. Maternal venous and umbilical cord arterial blood samples were collected immediately after delivery, and pro-BNP levels were measured using standard laboratory procedures. Maternal pro-BNP levels were significantly higher in the COVID-19 group, whereas cord blood pro-BNP levels did not differ significantly between groups. Obstetric and neonatal outcomes were similar. These findings suggest that prior COVID-19 infection may cause subclinical maternal cardiac stress, while placental mechanisms may protect the fetus from cardiac involvement.

این مطالعه هدف داشت سطوح پرو-پروتئین B-نوروترپین (pro-BNP) در خون مادر و کبد خونی را در زنان باردار با سابقه عفونت کووید-۱۹ مقایسه کرده و اثرات بالقوه قلبی را در حین بارداری ارزیابی کند. این مطالعه مورد-شاهد پیش‌رو بین ژانویه و اکتبر ۲۰۲۳ انجام شد و ۸۸ زن باردار شامل شد: ۴۴ نفر با سابقه عفونت کووید-۱۹ پس از دومین ترم و ۴۴ نفر شاهد سالم. زنان دارای دیابت، فشار خون بالا، بیماری قلبی یا complications بارداری شدید حذف شدند. نمونه‌های خون مادرین وسیلی و کبد خونی آروریال بلافاصله پس از زایمان جمع‌آوری شدند و سطوح pro-BNP با روش‌های استاندارد آزمایشگاهی اندازه‌گیری شدند. سطوح pro-BNP مادر در گروه کووید-۱۹ به طور معناداری بالاتر بود، در حالی که سطوح pro-BNP خون کبد خونی بین گروه‌ها تفاوت معناداری نداشت. نتایج زایشی و نوزادی مشابه بود. این یافته‌ها نشان می‌دهند که عفونت قبلی کووید-۱۹ ممکن است باعث استرس قلبی زیرسطحی مادر شود، در حالی که مکانیسم‌های جفت ممکن است جنین را در برابر درگیری قلبی محافظت کند.
