BACKGROUND: External ventricular drains (EVDs) are commonly used in neurosurgery but carry a risk of ventriculostomy-related infection. Routine cerebrospinal fluid (CSF) sampling is commonly performed for surveillance, although its clinical value and potential risks are still uncertain. METHODS: A systematic review was performed following PRISMA 2020 guidelines. PubMed was searched from inception to identify studies assessing CSF sampling strategies and/or clinical and laboratory screening for infection in patients with EVDs. Eligible studies included retrospective and prospective cohorts, case-control, and case series reporting infection-related outcomes. Data were extracted and categorized by causal relevance. Due to heterogeneity in studies, findings were summarized descriptively. RESULTS: Fourteen studies were included, covering various study designs and sampling methods. Only 2 studies directly compared routine versus clinically indicated CSF sampling and found no benefit of routine daily sampling for detecting or reducing infections. Most studies were noncomparative and demonstrated the limited predictive value of routine clinical and laboratory parameters. Indirect evidence suggested that catheter duration, system manipulation, and patient-related factors were more strongly associated with infection risk than sampling frequency. Infection rates across studies ranged from 4.1% to 16.3%, with considerable variation in definitions and methodologies. CONCLUSION: Routine CSF sampling is not supported by high-quality evidence as a strategy to improve detection or prevent EVD-related infections. A selective, clinically guided approach may be more appropriate, with emphasis on reducing catheter manipulation and optimizing overall catheter management. Further prospective studies with standardized protocols are necessary to clarify the role of CSF sampling strategies.

Exploring Proximal and Distal Ultrasound-Guided Greater Occipital Nerve Block Approaches in Migraine: A Systematic Review and Meta-Analysis.

OBJECTIVE: To evaluate the effectiveness and safety of proximal and distal ultrasound-guided greater occipital nerve block (US-GONB) in patients with migraine. METHODS: PubMed, Scopus, Web of Science, and the Cochrane Library were searched up to July 2025. Studies assessing US-GONB in migraine were included. Data were pooled using a random-effects model and expressed as mean difference (MD) with 95% confidence intervals (CI). Subgroup …

Applications of acellular dermal matrix in spine surgery: A systematic review.

Acellular dermal matrix (ADM) is a tissue-engineered biological scaffold widely adopted in plastic reconstructive surgery, yet its application in spine surgery remains largely underexplored. No prior systematic review has synthesized the available evidence. A systematic search of PubMed, Embase, Cochrane CENTRAL, Web of Science, and Scopus was performed from inception through 31 July 2026, following PRISMA 2020 guidelines (PROSPERO: CR…

The efficacy of surgical interventions for trigeminal neuralgia in multiple sclerosis: A systematic review.

Trigeminal neuralgia affects approximately 3% of patients with multiple sclerosis - a far higher prevalence than in the general population. The associated pain often proves refractory to medication, prompting consideration of neurosurgical intervention. The variable efficacy, tolerability, and durability of available procedures, compounded by factors such as multiple sclerosis subtype and neuroimaging findings, require evidence to guid…

Volumetric analysis of radiographic changes after stereotactic radiosurgery for treatment of non-responding brain metastases.

PURPOSE: Stereotactic radiosurgery (SRS) effectively treats brain metastases, yet some lesions ultimately require post-SRS surgery due to local failure. We examined whether the use of volumetric changes in FLAIR hyperintensity and/or necrosis in conjunction with enhancing volumetric changes after Gamma Knife SRS correlate with pathological outcome at subsequent surgery. METHODS: We queried our institutional database for patients who re…
