چکیده اصلی

The brain is never truly at rest. Even in the absence of external stimuli, the central nervous system continuously monitors and regulates visceral organs. However, it remains unclear to what extent this interoceptive signaling shapes the brain's resting state networks. Here, we tested the hypothesis that resting-state functional connectivity is actively sustained by visceral inputs. Using functional magnetic resonance imaging in 34 anesthetized rats, we identified a cohesive "interoception network" characterized by dense, reciprocal functional connectivity among the dorsal vagal complex, thalamus, hypothalamus, amygdala, insula, and cingulate cortex. The topology of this network was not static but dynamically coupled to the physiological state. Functional integration was robust in the digestive (fed) phase but significantly attenuated in the inter-digestive (fasted) phase. The network relied on the integrity of the vagus nerve. When vagal signaling was eliminated by bilateral cervical vagotomy, the network effectively disintegrated. These findings demonstrate that resting-state functional connectivity is not an intrinsic property of the isolated brain but an embodied phenomenon that relies on continuous neural signaling between the brain and viscera.

Exploring Proximal and Distal Ultrasound-Guided Greater Occipital Nerve Block Approaches in Migraine: A Systematic Review and Meta-Analysis.

OBJECTIVE: To evaluate the effectiveness and safety of proximal and distal ultrasound-guided greater occipital nerve block (US-GONB) in patients with migraine. METHODS: PubMed, Scopus, Web of Science, and the Cochrane Library were searched up to July 2025. Studies assessing US-GONB in migraine were included. Data were pooled using a random-effects model and expressed as mean difference (MD) with 95% confidence intervals (CI). Subgroup …

Applications of acellular dermal matrix in spine surgery: A systematic review.

Acellular dermal matrix (ADM) is a tissue-engineered biological scaffold widely adopted in plastic reconstructive surgery, yet its application in spine surgery remains largely underexplored. No prior systematic review has synthesized the available evidence. A systematic search of PubMed, Embase, Cochrane CENTRAL, Web of Science, and Scopus was performed from inception through 31 July 2026, following PRISMA 2020 guidelines (PROSPERO: CR…

The efficacy of surgical interventions for trigeminal neuralgia in multiple sclerosis: A systematic review.

Trigeminal neuralgia affects approximately 3% of patients with multiple sclerosis - a far higher prevalence than in the general population. The associated pain often proves refractory to medication, prompting consideration of neurosurgical intervention. The variable efficacy, tolerability, and durability of available procedures, compounded by factors such as multiple sclerosis subtype and neuroimaging findings, require evidence to guid…

Volumetric analysis of radiographic changes after stereotactic radiosurgery for treatment of non-responding brain metastases.

PURPOSE: Stereotactic radiosurgery (SRS) effectively treats brain metastases, yet some lesions ultimately require post-SRS surgery due to local failure. We examined whether the use of volumetric changes in FLAIR hyperintensity and/or necrosis in conjunction with enhancing volumetric changes after Gamma Knife SRS correlate with pathological outcome at subsequent surgery. METHODS: We queried our institutional database for patients who re…
