چکیده اصلی

Central neurocytoma is a rare intraventricular tumor with favorable prognosis following gross total resection (GTR). However, the role of Ki67-MIB1 values in the postoperative decision making-process of treatment, as well as the indication for adjuvant radiotherapy, remain debated. Medical record data of patients with confirmed diagnosis of central neurocytoma who underwent surgical resection at Department of Neurosurgery of "Ospedale del Mare" in Naples - Italy, between January and December 2022, were retrospectively reviewed. Inclusion criteria were adult patients (> 18 years old), minimum follow up 42 months, complete demographic, clinical, neuroradiological and histopathological data, type of management, complications and outcome, time to recurrence, follow-up duration. Five adult patients were enrolled according to the inclusion criteria. Most patients were women (80%), with mean age 24 years old. Main presenting symptoms were headache and visual deficit. Lesions always involved lateral ventricles, with extension to the third ventricle in all but one cases. Transcortical approach to the ipsilateral lateral ventricle with extension to the third ventricle was the main adopted surgical route. GTR was achieved in all cases without postoperative complications nor postoperative new onset neurological deficits. No patients required ventricular shunting procedures for hydrocephalus during the surveillance period. Follow-up ranged from 43 to 54 months. Ki67-MIB1 values ranged from 5% to 10%. No patients underwent adjuvant radiotherapy. Our findings suggest that GTR alone may be sufficient even in patients with moderately elevated Ki-67. A tailored, risk-adapted postoperative strategy is recommended rather than routine adjuvant treatment.

در این مطالعه ۵ بیمار بالغ با نورونوسیتومای مرکزی بررسی شدند که در تمام موارد GTR با موفقیت انجام شد. هیچ بیماری نیازی به رادیوتراپی کمکی یا پروتز ventricles نداشتند. یافته‌ها نشان می‌دهد که GTR به تنهایی ممکن است کافی باشد، به‌ویژه در بیماران با Ki-67 متوسط.

نورونوسیتومای مرکزی یک تومار داخل‌بطری نادر است که پیش‌آگهی مطلوبی پس از برداشت کامل (GTR) دارد. با این حال، نقش مقادیر Ki67-MIB1 در فرآیند تصمیم‌گیری پس از جراحی و همچنین نشانگر رادیوتراپی کمکی همچنان مورد بحث است. داده‌های سوابق پزشکی بیماران مبتلا به نورونوسیتومای مرکزی که در دپارتمان جراحی مغز و اعصاب «اُسپیداله دل ماره» در ناپل، ایتالیا، بین ژانویه و دسامبر ۲۰۲۲ تحت عمل جراحی قرار گرفتند، به صورت بازنگری شده بررسی شدند. معیارهای ورود شامل بیماران بالغ (>۱۸ سال)، پیگیری حداقل ۴۲ ماهه، داده‌های کامل دموگرافیک، بالینی، نوارعصب‌شناسی و بافت‌شناسی، نوع مدیریت، عوارض و نتیجه، زمان بازگشت تومار و مدت پیگیری بودند. پنج بیمار بالغ طبق معیارهای ورود ثبت شدند. اکثر بیماران زن (۸۰٪) و میانگین سنی ۲۴ سال بودند. علائم اصلی سردرد و نقص بینایی بود. توده‌ها همیشه ventricles جانبی را درگیر می‌کردند و در همه موارد به جز یک مورد به ventricles سوم گسترش داشتند. رویکرد ترانس‌کورتیکال به ventricles جانبی هم‌سو با گسترش به ventricles سوم مسیر اصلی جراحی بود. GTR در تمام موارد بدون عوارض پس از جراحی یا نقص عصبی جدید حاصل شد. هیچ بیماری برای پروتز ventricles در پی آبسه مغزی نیازی نبود. پیگیری از ۴۳ تا ۵۴ ماه متغیر بود. مقادیر Ki67-MIB1 بین ۵٪ تا ۱۰٪ متغیر بود. هیچ بیماری رادیوتراپی کمکی دریافت نکرد. یافته‌های ما نشان می‌دهد که GTR به تنهایی ممکن است حتی در بیماران با Ki-67 متوسط بالا کافی باشد. توصیه می‌شود به جای درمان کمکی روتین، یک استراتژی پس از جراحی متناسب با ریسک تعیین شود.

روش پژوهش

داده‌های سوابق پزشکی ۵ بیمار بالغ که در سال ۲۰۲۲ تحت عمل جراحی قرار گرفتند، به صورت بازنگری شده بررسی شدند. معیارهای ورود شامل پیگیری حداقل ۴۲ ماهه و داده‌های کامل بالینی و بافت‌شناسی بود.

محدودیت‌ها

مطالعه دارای نمونه‌گیری کوچک (۵ مورد) و بازه پیگیری محدود (حداکثر ۵۴ ماه) است. نقش رادیوتراپی کمکی در این مطالعه بررسی نشد.
