چکیده اصلی

INTRODUCTION: The nurse-patient relationship is crucial for conscious mechanically ventilated patients in the intensive care unit (ICU). While critical illness, immobilisation, and communication barriers challenge this relationship, the knowledge of its enactment in practice remains limited. AIM: The aim of this study was to explore how the nurse-patient relationship with conscious mechanically ventilated patients in the ICU is enacted in clinical practice. METHODS: An ethnographic design was employed. Twenty-five participant observations supported by informal interviews (94 h) were conducted with nurses caring for conscious, nondelirious, mechanically ventilated adults in ICUs in Denmark over a 10-month period (May 2024-March 2025). Data were analysed using inductive reflexive thematic analysis. Reporting followed the Consolidated criteria for Reporting Qualitative (COREQ) guidelines. RESULTS: Relational orientation on a continuum, generated as an overarching theme, unfolds through the following subthemes: (i) The importance of navigating attentiveness in patient care; (ii) advocating for patients as a central aspect of establishing trust; (iii) active involvement of patients as a central aspect of care; and (iv) touch as a central aspect of nonverbal communication. CONCLUSIONS: The nurse-patient relationship with conscious mechanically ventilated ICU patients was enacted along a continuum ranging from technical-instrumental to relational focus, where safety, trust, and collaboration are essential. Nurses dynamically navigate this continuum, balancing attentiveness, advocacy, involvement, and touch with clinical and technical competence while remaining sensitive to patients' pace and capacity as well as managing contextual factors, such as technology and interruptions. This emphasises that nurses' responsibility and commitment to the nursepatient relationship is fundamental to provide quality care.

مقدمه: رابطه پرستار-بیمار برای بیماران آگاه و وصل به دستگاه تنفس مصنوعی در مراکز مراقبت‌های ویژه (ICU) حیاتی است. علائم بیماری حاد، ایستایی و موانع ارتباطی چالش‌هایی برای این رابطه ایجاد می‌کنند، اما دانش در مورد نحوه اجرای آن در عمل محدود است. هدف: هدف این مطالعه بررسی نحوه اجرای رابطه پرستار-بیمار با بیماران آگاه و وصل به دستگاه تنفس مصنوعی در ICU در عمل بالینی بود. روش‌ها: از یک طراحی اثنوگرافیک استفاده شد. ۲۵ مشاهده مشارکتی همراه با مصاحبه‌های غیررسمی (۹۴ ساعت) با پرستاران مراقبت از بیماران بالغ آگاه، بدون دلیریوم و وصل به دستگاه تنفس مصنوعی در واحدهای ICU دانمارک در طی یک دوره ۱۰ ماهه (مه ۲۰۲۴ - مارس ۲۰۲۵) انجام شد. داده‌ها با استفاده از تحلیل موضوعی بازتابی استنتاجی تحلیل شدند. گزارش‌دهی طبق دستورالعمل‌های یکپارچه برای گزارش پژوهش‌های کیفی (COREQ) انجام شد. نتایج: جهت‌گیری رابطه‌ای در یک طیف، به عنوان یک موضوع فرعی، از طریق زیرموضوع‌های زیر unfold می‌شود: (i) اهمیت هدایت توجه در مراقبت از بیمار؛ (ii) دفاع از بیماران به عنوان یک جنبه مرکزی برای ایجاد اعتماد؛ (iii) درگیر شدن فعال بیماران به عنوان یک جنبه مرکزی مراقبت؛ و (iv) لمس به عنوان یک جنبه مرکزی ارتباط غیرکلامی. نتیجه‌گیری: رابطه پرستار-بیمار با بیماران ICU آگاه و وصل به دستگاه تنفس مصنوعی در یک طیف از تمرکز فنی-ابزاری تا تمرکز رابطه‌ای اجرا شد، جایی که ایمنی، اعتماد و همکاری ضروری هستند. پرستاران به طور پویا در این طیف حرکت می‌کنند، توجه، دفاع، درگیری و لمس را با توانایی بالینی و فنی متعادل می‌کنند، در حالی که به سرعت بیمار و ظرفیت او حساس هستند و عوامل زمینه‌ای مانند تکنولوژی و مختصات را مدیریت می‌کنند. این تأکید می‌کند که مسئولیت و تعهد پرستاران به رابطه پرستار-بیمار برای ارائه مراقبت با کیفیت بنیادین است.
