چکیده اصلی

BACKGROUND: Pain management in critically ill patients with brain injury is a clinical and ethical concern in critical care settings that is complicated by altered consciousness and communication barriers. AIM: To identify, synthesise and categorise methods for effective pain assessment in critically ill, non-verbal patients with brain injury in critical care settings. DESIGN: A systematic review following PRISMA guidelines, registered in PROSPERO, included 13 articles that examined pain among those patients. RESULTS: Validated assessment tools, such as the Critical Care Pain Observation Tool and the Behavioural Pain Scale, remain the gold standard for pain assessment in patients with brain injury. The most universal behavioural indicators of pain in these patients are frowning, sudden eye opening and limb flexion. However, behavioural indicators are often blunted by the level of consciousness, sedation or severity of injury. Conversely, physiological parameters lack diagnostic specificity and might be confounded by autonomic nervous system instability or medications. Emerging advanced technologies offer an objective approach to the nociception process. CONCLUSION: Clinicians should be cautious while assessing pain in brain-injured patients; a low pain score may be a false negative caused by the inability to react rather than an actual absence of pain. It is recommended to adopt a multi-modal approach for pain assessment in critically ill patients with brain injury. RELEVANCE TO CLINICAL PRACTICE: The findings may inform pain assessment protocols in critically ill non-verbal patients with brain injury, suggesting that using an isolated approach is inadequate and inaccurate.

Bridging the Awareness-Action Gap in Disaster Preparedness: A Qualitative Study of Immersive Virtual Reality Learning Among Nursing Students.

BACKGROUND: Disasters increasingly challenge healthcare systems, highlighting the need to strengthen preparedness among future healthcare professionals. Although disaster risk awareness is important, it does not consistently translate into preparedness behaviours, reflecting the awareness-action gap. Immersive virtual reality (VR) may enhance experiential disaster learning, but how it influences preparedness processes remains insuffici…

Comparison of the Validity and Reliability of Five Pressure Injury Risk Scales in Intensive Care.

BACKGROUND: The lack of consensus on the best pressure injury (PI) risk assessment tools demonstrates the need for further comparative research to identify the most effective options for intensive care unit (ICU) populations. AIM: This study compared the predictive validity and reliability of five PI risk assessment tools in adult ICU patients. STUDY DESIGN: A prospective cohort study was conducted with patients aged ≥ 18 years, admitt…

Although Zambia has expanded critical care services over the last 15 years, national evidence regarding workforce capacity, infrastructure and service provision remains limited. AIMS: To evaluate critical care workforce, service provision and patient case mix across Ministry…

Electrical Impedance Tomography-Guided Precision Nursing in an Older Patient With Septic Shock and ARDS: A Case Report.

Septic shock complicated by acute respiratory distress syndrome (ARDS) and multiple organ dysfunction syndrome (MODS) presents a fundamental therapeutic conflict: shock demands aggressive fluid resuscitation, whereas lung protection requires fluid restriction and a negative fluid balance. We report the precision nursing management of a 79-year-old patient with septic shock, ARDS and MODS who received invasive mechanical ventilation and…
