چکیده اصلی

BACKGROUND: Childbirth during the COVID-19 pandemic involved infection-control measures and changes in maternity care that may have increased psychological stress and affected early mother-infant bonding, particularly among women infected at delivery. OBJECTIVE: This study aimed to compare postpartum depressive symptoms, anxiety levels, and mother-infant bonding among women with positive versus negative COVID-19 PCR test results at the time of delivery. METHODS: The study included 107 postpartum women (53 COVID-19 positive, 54 COVID-19 negative). Participants completed a sociodemographic form, the Edinburgh Postnatal Depression Scale, the State-Trait Anxiety Inventory and the Postpartum Bonding Questionnaire within the first few days of postpartum hospitalization. RESULTS: COVID-19-positive women had significantly higher depressive symptoms, state anxiety, and bonding difficulty scores. In both groups, depressive and anxiety symptoms were positively correlated with bonding problems, with stronger associations in the positive group. In multivariate analyses, depressive symptoms and particularly state anxiety remained independently associated with bonding difficulties, whereas COVID-19 status was no longer significantly associated after adjustment for anxiety. CONCLUSIONS: This study emphasizes the importance of considering care practices that support both maternal mental health and healthy mother-infant bonding when planning maternity services during public health emergencies.

این مطالعه مقطعی نشان داد که زنان دارای عفونت کووید-۱۹ در زمان زایمان علائم افسردگی و اضطراب بالاتر و دشواری بیشتری در بازدارندگی مادر-نوزاد را تجربه می‌کنند. همبستگی بین علائم روان‌شناختی و مشکلات بازدارندگی در گروه عفونت‌دار قوی‌تر بود. پس از تعدیل، اضطراب حال به طور مستقل با دشواری‌های بازدارندگی مرتبط باقی ماند.

زمینه: زایمان در دوران همه‌گیری کووید-۱۹ با اقدامات کنترل عفونت و تغییرات در مراقبت‌های مامایی همراه بوده که ممکن است استرس روان‌شناختی را افزایش داده و بازدارندگی زنان و نوزاد را در روزهای اول مختل کرده باشد. هدف: این مطالعه مقایسه علائم افسردگی پس از زایمان، سطح اضطراب و بازدارندگی مادر-نوزاد در زنان دارای نتیجه مثبت یا منفی تست PCR کووید-۱۹ در زمان زایمان را هدف قرار داد. روش‌ها: این مطالعه شامل ۱۰۷ زن پس از زایمان (۵۳ مثبت و ۵۴ منفی) بود. شرکت‌کنندگان فرم‌های جمعیت‌شناختی، مقیاس افسردگی پس از زایمان ادینبورگ، پرسشنامه اضطراب حال و ویژگی و پرسشنامه بازدارندگی پس از زایمان را در چند روز اول بستری پس از زایمان تکمیل کردند. نتایج: زنان مثبت کووید-۱۹ علائم افسردگی، اضطراب حال و نمرات دشواری در بازدارندگی را به طور معناداری بالاتر گزارش کردند. در هر دو گروه، علائم افسردگی و اضطراب با مشکلات بازدارندگی همبستگی مثبت داشتند، با همبستگی‌های قوی‌تر در گروه مثبت. در تحلیل‌های چندمتغیره، علائم افسردگی و به طور خاص اضطراب حال به طور مستقل با دشواری‌های بازدارندگی مرتبط باقی ماندند، در حالی که وضعیت کووید-۱۹ پس از تعدیل اضطراب دیگر ارتباط معناداری نداشت. نتیجه‌گیری: این مطالعه اهمیت در نظر گرفتن شیوه‌های مراقبتی را که هم سلامت روان مادر و هم بازدارندگی سالم مادر-نوزاد را پشتیبانی می‌کنند، در برنامه‌ریزی خدمات مامایی در بحران‌های بهداشت عمومی برجسته می‌کند.
