چکیده اصلی

Thoraco-lumbar burst fractures are a type of spinal injury that typically result from high-energy trauma. These fractures involve compression of the vertebral body with posterior displacement of bone fragments into the spinal canal. Treatment typically involves surgical stabilization with pedicle screw and rod instrumentation. The aim of the study was to evaluate the results of thoracolumbar burst fractures with incomplete neurological lesion stabilized by pedicle screw and rod. This was a prospective experimental study carried out at Bangladesh Medical University, Dhaka, Bangladesh from January 2019 to December 2019. A total of 15 patients with Thoraco-Lumbar Burst fracture aged over 15 years were included in the study. A detailed history of the selected patient was taken with mode of injury, time of injury, other history, then thorough general, local, neurological examination for proper assessment. Among the 15 cases, 12 cases (80.0%) were male and 3 cases (20.0%) were female. The age range of patient was from 17 55 years. Most commonly affected people were laborers 5(33.33%). Fall from height was the commonest cause of injury 11(73.30%). Most commonly level of injury was L1 07 (46.70%). Pedicle screw and rod used in all cases for long segment reconstruction above and below the involved level. According to Frankel grade preoperatively most common grade was C 10 and next common was B 5. Post operatively most was D 9 and next was E 4 and C-2. According to Denis et al pain scale most commonly 8 (53.34%) cases complain no pain and work scale most commonly 2(13.33%) cases were completely well and can perform normal work up to latest follow-up. Mean hospital stay was (13.27±3.54) days (10 20) days. Satisfactory results (excellent and good) were in 13(86.66%), fair 1(6.67%) and 1(6.67%) unsatisfactory (poor) results. The results of the study demonstrates that functional outcome of Thoraco-Lumbar burst fractures stabilization by pedicle screw and rod is an effective procedure.

شکستگی‌های توراکو-لومباری نوعی آسیب نخاعی هستند که معمولاً ناشی از ضربات انرژی بالا هستند. این شکستگی‌ها شامل فشردن بدنه مهره و جابجایی پس‌سری تکه‌های استخوان به داخل کانال نخاعی است. درمان معمولاً شامل پایدارسازی جراحی با ابزار پیچ و میله اسکله‌ای است. هدف این مطالعه ارزیابی نتایج شکستگی‌های توراکو-لومباری با آسیب عصبی ناقص که با پیچ و میله اسکله‌ای پایدارسازی شده‌اند بود. این یک مطالعه تجربی پیش‌رونده در دانشگاه پزشکی بنگلادش، داکا، بنگلادش از ژانویه ۲۰۱۹ تا دسامبر ۲۰۱۹ انجام شد. مجموعاً ۱۵ بیمار مبتلا به شکستگی توراکو-لومباری با سن بالای ۱۵ سال در مطالعه گنجانده شدند. ۱۲ مورد (۸۰.۰٪) مرد و ۳ مورد (۲۰.۰٪) زن بودند. سن بیماران بین ۱۷ تا ۵۵ سال بود. شایع‌ترین عامل آسیب سقوط از ارتفاع بود (۱۱ مورد، ۷۳.۳۰٪). شایع‌ترین سطح آسیب مهره L1 بود (۷ مورد، ۴۶.۷۰٪). در تمام موارد از پیچ و میله اسکله‌ای برای بازسازی طولانی‌بُعدی بالای و زیر سطح درگیر استفاده شد. بر اساس نمره فرانکل پیش از عمل، شایع‌ترین نمره C بود (۱۰ مورد) و نمره B بعدی ۵ مورد بود. پس از عمل، شایع‌ترین نمره D ۹ مورد و نمره E ۴ مورد و C ۲ مورد بود. بر اساس مقیاس درد دنیس، ۸ مورد (۵۳.۳۴٪) درد نداشتند و بر اساس مقیاس شغلی، ۲ مورد (۱۳.۳۳٪) کاملاً سالم بودند و می‌توانستند کار عادی انجام دهند تا آخرین پیگیری. میانگین بستری بیمارستان (۱۳.۲۷±۳.۵۴) روز بود (۱۰ تا ۲۰ روز). نتایج مطلوب (عالی و خوب) در ۱۳ مورد (۸۶.۶۶٪)، متوسط در ۱ مورد (۶.۶۷٪) و ناپسند (بد) در ۱ مورد (۶.۶۷٪) مشاهده شد. نتایج مطالعه نشان می‌دهد که پایدارسازی شکستگی‌های توراکو-لومباری با پیچ و میله اسکله‌ای یک روش مؤثر است.

روش پژوهش

این یک مطالعه تجربی پیش‌رونده بود که در دانشگاه پزشکی بنگلادش انجام شد. تاریخچه، معاینه عمومی و محلی و معاینه عصبی تمام بیماران ثبت شد.

محدودیت‌ها
