چکیده اصلی

BACKGROUND: Cervical cancer can be prevented and treated if detected early through regular screening. However, 88% of women aged 30 to 49 have never been screened in Tanzania. OBJECTIVE: This study explored facilitators and barriers to cervical cancer screening among women and healthcare providers in rural Kilimanjaro, Tanzania, using the Theoretical Domains Framework and the Capability, Opportunity, Motivation-Behavior model. METHODS: An exploratory qualitative design was employed; data were collected from March to October 2024. Six focus group discussions were conducted with 54 women aged 30-50 years, and twelve in-depth individual interviews with healthcare providers. Data were transcribed, coded, and thematically analyzed using QDA Miner Lite v2.0.8. RESULTS: Four main themes emerged, each with sub-themes: (1) knowledge about cervical cancer and screening (disparities in knowledge between screened and non-screened women; healthcare providers' knowledge and training needs; age and parity as knowledge mediators); (2) power of social influence (social facilitation; social barriers); (3) emotional and structural influences on cervical cancer screening (emotional barriers and coping; structural and logistical barriers); and (4) enhancing cervical cancer screening uptake (targeted community education and outreach; improving service accessibility). Barriers include limited knowledge, misconceptions, fear of pain and positive results, limited access to services, and a shortage of trained healthcare providers. Social support from families, peers, community leaders, and healthcare providers was identified as a strong facilitator. CONCLUSION: Increasing cervical cancer screening uptake in rural communities requires targeted, theory-informed interventions that incorporate education to enhance awareness, strengthen social support, and improve health services.

این مطالعه کیفی در تانزانیا، عوامل تسهیل‌کننده و موانع غربالگری سرطان cervix را در زنان و ارائه‌دهندگان مراقبت‌های بهداشتی روستایی بررسی کرد. موانع شامل دانش محدود، باورهای غلط و ترس از درد بودند، در حالی که حمایت اجتماعی و دسترسی به خدمات عوامل تسهیل‌کننده مهمی بودند. نتایج نشان می‌دهد که مداخلات آموزشی و بهبود دسترسی به خدمات برای افزایش پذیرش غربالگری در جوامع روستایی ضروری است.

زمینه: سرطان cervix را می‌توان با تشخیص زودهنگام و غربالگری منظم پیشگیری و درمان کرد. با این حال، ۸۸ درصد از زنان ۳۰ تا ۴۹ ساله در تانزانیا هرگز غربالگری نشده‌اند. هدف: این مطالعه با استفاده از چارچوب حوزه‌های نظری و مدل توانایی، فرصت، انگیزه-رفتار، عوامل تسهیل‌کننده و موانع غربالگری سرطان cervix را در زنان و ارائه‌دهندگان مراقبت‌های بهداشتی در منطقه روستایی کیلمنجارو، تانزانیا بررسی کرد. روش‌ها: از یک طراحی کیفی اکتشافی استفاده شد؛ داده‌ها از مارس تا اکتبر ۲۰۲۴ جمع‌آوری شدند. شش گروه بحث تمرکز با ۵۴ زن ۳۰ تا ۵۰ ساله و دوازده مصاحبه عمیق فردی با ارائه‌دهندگان مراقبت‌های بهداشتی برگزار شد. داده‌ها با استفاده از QDA Miner Lite v2.0.8 ترجمه، کدگذاری و تحلیل موضوعی شدند. نتایج: چهار موضوع اصلی با زیرموضوع‌های خود ظاهر شدند: (۱) دانش در مورد سرطان cervix و غربالگری (ناهمگونی دانش بین زنان غربالگری‌شده و غیرغربالگری‌شده؛ نیازهای آموزشی و دانش ارائه‌دهندگان مراقبت‌های بهداشتی؛ سن و باروری به عنوان میانجی‌های دانش)؛ (۲) قدرت تأثیر اجتماعی (تسهیل اجتماعی؛ موانع اجتماعی)؛ (۳) تأثیرات عاطفی و ساختاری بر غربالگری سرطان cervix (موانع عاطفی و سازگاری؛ موانع ساختاری و لجستیکی)؛ و (۴) افزایش پذیرش غربالگری سرطان cervix (آموزش و گسترش جامعه‌محور هدفمند؛ بهبود دسترسی به خدمات). موانع شامل دانش محدود، باورهای غلط، ترس از درد و نتایج مثبت، دسترسی محدود به خدمات و کمبود ارائه‌دهندگان مراقبت‌های بهداشتی آموزش‌دیده است. حمایت اجتماعی از خانواده‌ها، همسالان، رهبران جامعه و ارائه‌دهندگان مراقبت‌های بهداشتی به عنوان یک تسهیل‌کننده قوی شناسایی شد. نتیجه‌گیری: افزایش پذیرش غربالگری سرطان cervix در جوامع روستایی نیاز به مداخلات هدفمند و مبتنی بر نظری دارد که آموزش را برای ارتقای آگاهی، تقویت حمایت اجتماعی و بهبود خدمات بهداشتی در بر می‌گیرد.
