Nursing Students' Reports of Patient Safety Incidents and Reasons During Clinical Placements: A Secondary Analysis of the International Data.

چکیده اصلی

Clinical placements expose nursing students to patient safety incidents and provide important opportunities for learning about safe care. This study explored how nursing students in four countries recognized and interpreted patient safety incidents encountered or witnessed during clinical practice, including perceived contributing factors. A secondary qualitative content analysis was conducted using narrative data from 1442 undergraduate nursing students in the Czech Republic, Italy, Slovakia, and Türkiye. In total, 287 incident descriptions and 263 explanations of perceived reasons were analyzed using an inductive-deductive approach supported by descriptive frequency counts. Medication safety incidents and patient falls were most frequently reported, alongside infection prevention breaches, missed or inadequate care, communication failures, adverse outcomes, and environmental hazards. Students described incidents as multifactorial, involving patient and contextual factors, organizational and system conditions, and human and professional factors. Cross-country differences reflected patterns within the analyzed narratives rather than differences in incident prevalence or quality of care. The findings support practice-integrated patient safety education, effective supervision, structured reflection and debriefing, clear reporting pathways, and psychologically and emotionally safe, nonpunitive learning environments.

این مطالعه با تحلیل داده‌های ۱۴۴۲ دانشجوی پرستاری در چهار کشور، حوادث ایمنی بیمار و دلایل آن‌ها را بررسی کرد. حوادث دارویی و سقوط بیماران شایع‌ترین موارد بودند. دانشجویان عوامل حادثه را چندعاملی می‌دانستند. تفاوت‌ها بین کشورها بیشتر در نحوه توصیف حوادث بود تا در شیوع واقعی حوادث.

کارآموزی‌های بالینی دانشجویان پرستاری را در معرض حوادث ایمنی بیمار قرار می‌دهد و فرصت‌های مهمی برای یادگیری مراقبت ایمن فراهم می‌کند. این مطالعه بررسی کرد که دانشجویان پرستاری در چهار کشور چگونه حوادث ایمنی بیمار را که در حین تمرین بالینی مشاهده یا تجربه کرده‌اند، شناسایی و تفسیر می‌کردند، از جمله عوامل موثر در نظر گرفته شده. تحلیل کیفی محتوایی ثانویه با استفاده از داده‌های داستانی ۱۴۴۲ دانشجوی کارشناسی پرستاری در جمهوری چک، ایتالیا، اسلواکی و ترکیه انجام شد. در مجموع، ۲۸۷ توصیف حادثه و ۲۶۳ توضیح عوامل موثر در نظر گرفته شده با رویکرد استنتاجی-استنتاجی و با پشتیبانی شمارش توصیفی فراوانی تحلیل شدند. حوادث ایمنی دارویی و سقوط بیماران بیشترین گزارش‌ها را داشتند، به همراه نقض‌های پیشگیری از عفونت، مراقبت‌های نادیده گرفته شده یا ناکافی، شکست‌های ارتباطی، نتایج نامطلوب و خطرات محیطی. دانشجویان حوادث را چندعاملی توصیف کردند که شامل عوامل بیمار و زمینه‌ای، شرایط سازمانی و سیستم‌ها، و عوامل انسانی و حرفه‌ای می‌شدند. تفاوت‌های بین‌المللی الگوهای درون داستان‌های تحلیل شده را منعکس می‌کردند، نه تفاوت‌ها در شیوع حادثه یا کیفیت مراقبت. یافته‌ها آموزش ایمنی بیمار با ادغام در عمل، نظارت مؤثر، تفکر و بازنگری ساختاریافته، مسیرهای گزارش‌دهی شفاف و محیط‌های یادگیری امن روانی و عاطفی و تنبیهی را پشتیبانی می‌کنند.

روش پژوهش

تحلیل کیفی محتوایی ثانویه با رویکرد استنتاجی-استنتاجی بر روی داده‌های داستانی دانشجویان انجام شد.
