چکیده اصلی

BACKGROUND: Complications after pancreaticoduodenectomy are common and may be life-threatening. Management of the common bile duct stump may affect biliary pressure, drainage, and postoperative outcomes. This study seeks to compare open versus ligated bile duct stump management during pancreaticoduodenectomy. METHODS: This retrospective cohort included 125 patients who underwent pancreaticoduodenectomy between 2019 and 2025. Surgical outcomes and postoperative recovery were compared between the open-stump and ligation groups. Multivariable logistic regression identified associations between stump management and postoperative complications; biochemical indicators were also assessed. RESULTS: After adjustment, the opening group had significantly lower risks of pancreatic fistula (aOR = 0.18, 95%CI: 0.05 - 0.68, P = 0.011) and intra-abdominal and gastrointestinal bleeding (aOR = 0.09, 95%CI: 0.01 - 0.77, P = 0.028) versus the ligation group. The opening group also demonstrated a shorter surgical duration (315 minutes vs. 420 minutes, P < 0.001), reduced postoperative hospitalization (24 days vs. 30 days, P = 0.002), and lower costs (109,375 RMB vs. 137,518 RMB, P < 0.001). No significant changes were seen in liver function or coagulation markers. CONCLUSION: Keeping the common bile duct open markedly enhances perioperative management during pancreaticoduodenectomy, thus serving as a significant reference for refining surgical protocols.

این مطالعه توصیفی-آزمایشی ۱۲۵ بیمار را بررسی کرد که تحت جراحی پانکراتودوئنکتومی قرار گرفتند. نتایج نشان داد که باز نگه داشتن کانال صفرا در برابر پیوند آن، خطر فیسور پانکراس و خونریزی را کاهش می‌دهد و مدت جراحی و هزینه‌ها را کاهش می‌دهد. عملکرد کبد و انعقاد خون تغییر معناداری نداشتند.

پس از جراحی پانکراتودوئنکتومی، عوارض شایعی رخ می‌دهد که می‌تواند تهدیدکننده حیات باشد. مدیریت تکه کانال صفرا ممکن است بر فشار صفرا، تخلیه و نتایج پس از عمل تأثیر بگذارد. این مطالعه به مقایسه مدیریت باز کردن تکه کانال صفرا در برابر پیوند آن در حین جراحی پانکراتودوئنکتومی می‌پردازد. این مطالعه توصیفی-آزمایشی شامل ۱۲۵ بیمار بود که بین سال‌های ۲۰۱۹ تا ۲۰۲۵ جراحی پانکراتودوئنکتومی دریافت کردند. نتایج جراحی و بهبودی پس از عمل بین گروه باز کردن تکه کانال و گروه پیوند مقایسه شد. رگرسیون لجستیک چندمتغیره روابط بین مدیریت تکه کانال و عوارض پس از عمل را شناسایی کرد؛ همچنین شاخص‌های بیوشیمیایی ارزیابی شدند. پس از تعدیل، گروه باز کردن تکه کانال ریسک کمتر معناداری برای فیسور پانکراس (aOR = 0.18، 95%CI: 0.05 - 0.68، P = 0.011) و خونریزی intra-abdominal و gastrointestinal (aOR = 0.09، 95%CI: 0.01 - 0.77، P = 0.028) نسبت به گروه پیوند داشت. گروه باز کردن تکه کانال همچنین مدت جراحی کوتاه‌تری (۳۱۵ دقیقه در برابر ۴۲۰ دقیقه، P < 0.001)، بستری پس از عمل کوتاه‌تر (۲۴ روز در برابر ۳۰ روز، P = 0.002) و هزینه کمتر (۱۰۹،۳۷۵ یوان در برابر ۱۳۷،۵۱۸ یوان، P < 0.001) نشان داد. تغییرات معناداری در عملکرد کبد یا شاخص‌های انعقاد مشاهده نشد. حفظ باز بودن کانال صفرا مشترک به طور قابل توجهی مدیریت دوران عمل را در جراحی پانکراتودوئنکتومی بهبود می‌بخشد و به عنوان یک مرجع مهم برای اصلاح پروتکل‌های جراحی عمل می‌کند.
