چکیده اصلی

BACKGROUND: General practitioners (GPs) play an increasingly central role in the detection and management of breast cancer recurrence (BCR). Socioeconomic position (SEP) has been associated with both risk of BCR and mortality and may influence cancer awareness and beliefs, healthcare-seeking behaviour, referral patterns, and access to and use of healthcare. Nevertheless, knowledge on how SEP is related to BCR detection in general practice remains limited. AIM: To characterise patients with BCR and examine how SEP is associated with involvement of general practice in the pathway to recurrence diagnosis (index consultation), symptom presentation in general practice, and the GP's decision to initiate a diagnostic pathway. METHODS: In this nationwide retrospective cohort study, national register data on women diagnosed with BCR between 1 January 2022 and 31 May 2024 were combined with GP survey data from 324 Danish GPs. SEP was measured as educational level and cohabitation status. Associations with study outcomes were analysed by estimating prevalence ratios using Poisson regression models. RESULTS: Among 392 women with BCR, 52.8% had an index consultation in general practice, 37.2% presented with symptoms in general practice, and 43.6% had the diagnostic pathway initiated by the GP. Across all analyses, no statistically significant associations were found between SEP and the investigated outcomes. CONCLUSION: The findings suggest no substantial socioeconomic disparities in the likelihood of having an index consultation in general practice, presenting with symptoms in general practice, or having the diagnostic pathway initiated by the GP when presenting with symptoms.

این مطالعه کوهورت سراسری، رابطه بین موقعیت اقتصادی-اجتماعی و درگیری پزشکان عمومی در تشخیص بازگشت سرطان پستان را بررسی کرد. داده‌های ملی و نظرسنجی پزشکان عمومی از ۳۹۲ زن استفاده شد. نتایج نشان داد که هیچ اختلاف معناداری اقتصادی-اجتماعی در میزان مشاوره، ظهور علائم یا آغاز مسیر تشخیزی توسط پزشکان عمومی وجود ندارد.

پزشکان عمومی (GPs) نقشی مرکزی در تشخیص و مدیریت بازگشت سرطان پستان (BCR) ایفا می‌کنند. موقعیت اقتصادی-اجتماعی (SEP) با خطر BCR و مرگ‌ومیر مرتبط است و ممکن است بر آگاهی و باورهای بیماران، رفتار جستجوی خدمات درمانی، الگوهای ارجاع و دسترسی به مراقبت‌ها تأثیر بگذارد. هدف این مطالعه، توصیف بیماران با BCR و بررسی رابطه SEP با درگیری پزشکان عمومی در مسیر تشخیص (مشاوره شاخص)، ظهور علائم در مراکز درمانی عمومی و تصمیم پزشکان برای آغاز مسیر تشخیزی بوده است. در این مطالعه کوهورت بازگشتی سراسری، داده‌های ثبت ملی زنانی که بین ۱ ژانویه ۲۰۲۲ و ۳۱ مه ۲۰۲۴ تشخیص BCR دریافت کرده‌اند، با داده‌های نظرسنجی از ۳۲۴ پزشک عمومی دانمارکی ترکیب شد. SEP بر اساس سطح تحصیلات و وضعیت هم‌سکونت سنجیده شد. روابط با نتایج با برآورد نسبت‌های شیوع با استفاده از مدل‌های رگرسیون پواسون تحلیل شد. در میان ۳۹۲ زن با BCR، ۵۲.۸٪ مشاوره شاخص در مراکز درمانی عمومی داشتند، ۳۷.۲٪ علائم را در مراکز درمانی عمومی نشان دادند و ۴۳.۶٪ مسیر تشخیزی توسط پزشکان عمومی آغاز شد. در تمام تحلیل‌ها، هیچ رابطه معناداری بین SEP و نتایج بررسی شده یافت نشد.

روش پژوهش

مطالعه کوهورت بازگشتی سراسری با استفاده از داده‌های ثبت ملی زنان تشخیص داده شده BCR (۱ ژانویه ۲۰۲۲ تا ۳۱ مه ۲۰۲۴) و نظرسنجی ۳۲۴ پزشک عمومی دانمارکی. SEP با رگرسیون پواسون و نسبت‌های شیوع تحلیل شد.
