چکیده اصلی

Dental implant therapy has become an increasingly widespread option for rehabilitation worldwide, with a substantial rise in both the number of implants placed and the patients seeking this type of treatment. As a result, the prevalence of biological and mechanical complications has also increased. Peri-implant inflammatory diseases - especially peri-implantitis, which involves progressive bone loss - have emerged as a major clinical challenge, as they can ultimately lead to the loss of the implant and the associated restoration, imposing significant financial and health burdens on patients. The primary etiological factor of peri-implantitis is the accumulation of bacterial biofilm. The condition most commonly occurs in patients with a history of unsuccessfully treated periodontal disease. Nevertheless, several additional risk factors contribute to its development, including insufficient alveolar bone support, weak peri-implant tissue, the lack of supportive periodontal and peri-implant maintenance therapy following implant placement. To ensure long-term treatment success, prevention and regular maintenance are crucial. Comprehensive management includes meticulous preoperative planning, ideal implant positioning, prosthetic designs that facilitate effective oral hygiene, and continuous professional supportive therapy. The following case report demonstrates that even in severe, full-arch peri-implantitis cases, favorable outcomes can be achieved through a multidisciplinary therapeutic approach when followed by strict maintenance. Long-term follow-up highlights that with proper rehabilitation and strong patient compliance, implant function can be preserved even in the presence of compromised peri-implant hard tissue environment.
