RATIONALE: Neonatal gastrointestinal perforation (NGP) is a life-threatening condition requiring urgent intervention. While exploratory laparotomy remains the standard treatment, conservative management offers an alternative when surgery is not feasible. This case demonstrates the successful use of comprehensive conservative therapy in a high-risk infant with NGP and suspected intestinal stenosis. PATIENT CONCERNS: A 6-day-old, small-for-gestational-age male infant presented with vomiting, abdominal distension, and respiratory distress. Clinical deterioration led to the diagnosis of pneumoperitoneum, sepsis, and suspected necrotizing enterocolitis. DIAGNOSES: NGP secondary to suspected necrotizing enterocolitis, with complications including septic shock, purulent meningitis, scrotal abscess, zinc deficiency, and malnutrition. INTERVENTIONS: Following parental refusal of laparotomy, conservative management was initiated, including peritoneal drainage, gastrointestinal decompression, broad-spectrum antibiotics, inotropic support, and parenteral nutrition. Nutritional support included zinc supplementation and gradual enteral feeding. OUTCOMES: The infant survived after 134 days of hospitalization, despite multiple complications. Long-term follow-up at 8 months revealed neurodevelopmental delays (developmental quotient 61), though physical growth was adequate with successful transition to complementary feeding. LESSONS: This case confirms that conservative management with peritoneal drainage and intensive multidisciplinary support can be a viable option for NGP, even in complex scenarios where surgery is not possible. It underscores the importance of vigilant monitoring, nutritional management, and awareness of potential long-term neurodevelopmental sequelae in such cases.

Choosing Not to Eat Meat: What Characterizes Independently Vegetarian Children?

A growing body of research has aimed to map the characteristics of extraordinary altruists-such as those who have donated a kidney to a stranger. However, this research has primarily focused on adults, and we know little about the development of extraordinary altruism. While children cannot donate their organs to strangers nor large portions of their income to charity, they can choose not to eat animals. In this study we examine indepe…

Essential Oral Single Nutritional Therapy Products for Inherited Metabolic Diseases: Evidence and Consensus Assessment Using a Modified Delphi Method.

Nutritional therapy is critical in managing inherited metabolic diseases (IMDs), and includes specialized diets and single nutritional therapy products (sNTPs) such as vitamins, cofactors, and amino acids. Many sNTPs function as medicines, but are regulated as food (e.g., food supplements), which can limit access, reimbursement, and consistent supply. Essential medicines, as defined by World Health Organization (WHO), meet key healthca…

Feeding a low-protein diet exacerbates the susceptibility to Citrobacter rodentium and dextran sulfate sodium-induced intestinal injury in mice.

Globally, almost half of all early childhood deaths are linked to severe undernutrition, herein referred to as severe malnutrition. Mortality in severely malnourished children is often attributed to common infectious diseases, including enteric infections. It has been proposed that impaired intestinal barrier function contributes to mortality, but direct evidence is limited and thus there exists a need to develop improved preclinical m…

Higher adherence to a Nordic diet is associated with reduced risk of mild depressive symptoms: a population-based analysis.

PURPOSE: Although the Mediterranean diet has been widely studied in relation to depression, less is known about the associations between other diets, such as the Nordic Diet, and depressive symptoms and clinical depression. The aim was to examine whether adherence to a Nordic diet (assessed with the modified Nordic Diet Index, mNDI) is associated with depressive symptoms and clinical depression in the Finnish Health 2000-2011 survey.
