چکیده اصلی

OBJECTIVE: This paper aims to assess the clinical effectiveness of combining distal first metatarsal biplanar Chevron osteotomy with either midshaft Scarf or proximal Lapidus osteotomy in treating moderate-to-severe hallux valgus. METHODS: This retrospective study included patients with moderate-to-severe hallux valgus treated with Chevron + Scarf or Chevron + Lapidus osteotomy and followed for at least 6 months. Outcomes included VAS, HVA, IMA, DMAA, AOFAS score, SF-36, osteotomy healing time, full weight-bearing time, and complications. RESULTS: Eighty-eight patients were included: 49 in the Chevron + Scarf group and 39 in the Chevron + Lapidus group. The Chevron + Scarf group had lower VAS scores at 1 week and 1 month postoperatively (P < 0.05), whereas the Chevron + Lapidus group achieved lower HVA and IMA at 3 and 6 months (P < 0.05). Healing time and time to full weight-bearing were shorter in the Chevron + Scarf group (P < 0.05). Improvements in DMAA, AOFAS, and SF-36 scores and complication rates were comparable between groups (P > 0.05). CONCLUSION: Both osteotomy strategies effectively correct moderate-to-severe hallux valgus. Each combination has relative advantages, and surgical choice should be tailored to the patient's anatomical and functional needs.

هدف: این مقاله ارزیابی اثربخشی بالینی ترکیب استخوان‌برش دوپلانار شیکون دال (Chevron) با استخوان‌برش میانی اسکارف یا استخوان‌برش پروکسیمال لپیدوس در درمان والگوس متوسط تا شدید است. روش‌ها: این مطالعه بازگشتی شامل بیماران با والگوس متوسط تا شدید که با استخوان‌برش شیکون + اسکارف یا شیکون + لپیدوس درمان شده و حداقل ۶ ماه دنبال شده‌اند. نتایج شامل نمره درد (VAS)، زاویه هالوکس والگوس (HVA)، زاویه درج میانی آلو (IMA)، زاویه درج میانی متاتارسال (DMAA)، نمره AOFAS، نمره SF-36، زمان بهبودی استخوان‌برش، زمان تحمل وزن کامل و عوارض بود. یافته‌ها: ۸۸ بیمار شامل شد: ۴۹ نفر در گروه شیکون + اسکارف و ۳۹ نفر در گروه شیکون + لپیدوس. گروه شیکون + اسکارف نمرات پایین‌تری درد (VAS) در ۱ هفته و ۱ ماه پس از عمل داشت (P < 0.05)، در حالی که گروه شیکون + لپیدوس زاویه‌های پایین‌تری HVA و IMA در ۳ و ۶ ماه به دست آورد (P < 0.05). زمان بهبودی و زمان تحمل وزن کامل در گروه شیکون + اسکارف کوتاه‌تر بود (P < 0.05). بهبود در نمرات DMAA، AOFAS و SF-36 و نرخ عوارض بین گروه‌ها قابل مقایسه بود (P > 0.05). نتیجه‌گیری: هر دو استراتژی استخوان‌برش به طور مؤثر والگوس متوسط تا شدید را اصلاح می‌کنند. هر ترکیب مزایای نسبی دارد و انتخاب جراحی باید متناسب با نیازهای آناتومیک و عملکردی بیمار باشد.
