OBJECTIVE: Myasthenia gravis (MG) is a chronic autoimmune disorder affecting the neuromuscular junction, characterized by fluctuating muscle weakness. Pregnancy in women with MG poses unique challenges, as physiological and immunological changes may influence disease activity and obstetric outcomes. CASE REPORT: A 25-year-old primigravida with initially ocular myasthenia gravis that progressed to generalized disease during pregnancy presented for delivery at 37 weeks gestation. The patient had initiated regular pyridostigmine therapy at the time of initial diagnosis one year prior to conception and remained free of exacerbations until the current pregnancy. Multidisciplinary coordination between obstetric, neurology, and anesthesia teams was established early. A planned cesarean delivery under spinal anesthesia was performed to minimize maternal respiratory fatigue and avoid myasthenic crisis. Both mother and neonate had favorable outcomes, and no postpartum exacerbation occurred. CONCLUSION: Optimal outcomes can be achieved with early diagnosis, monitoring and adequate treatment with a multidisciplinary approach. Delivery decisions based on respiratory assessment and the overall disease stability are crucial in preventing crisis and ensure both neonatal and maternal safety.

یک زن ۲۵ ساله باردار اول بار با مایستنی گراویس که به بیماری عمومی پیشرفت کرد، در ۳۷ هفته بارداری سزارین تحت بیهوشی نخاعی زایمان کرد. بیمار تحت درمان پیروستیگمین بود و با نظارت چندرشته‌ای، مادر و نوزاد نتایج مطلوبی داشتند.

  • سزارین تحت بیهوشی نخاعی برای کاهش خستگی تنفسی و جلوگیری از بحران توصیه شده است.

هدف: مایستنی گراویس (MG) یک اختلال خودایمنی مزمن است که بر اتصال عصب-عضله تأثیر می‌گذارد و با ضعف متغیر عضلانی مشخص می‌شود. بارداری در زنان دارای MG چالش‌های منحصر‌به‌فردی ایجاد می‌کند، زیرا تغییرات فیزیولوژیک و ایمنی ممکن است فعالیت بیماری و نتایج زایشی را تحت تأثیر قرار دهند. گزارش مورد: یک ۲۵ ساله باردار اول بار که ابتدا دارای مایستنی گراویس چشمی بود و در طول بارداری به بیماری عمومی پیشرفت کرد، در ۳۷ هفته بارداری برای زایمان مراجعه کرد. بیمار در زمان تشخیص اولیه یک سال قبل از بارداری شروع به درمان منظم پیروستیگمین کرده بود و تا بارداری فعلی بدون تشدید دچار تشدید نشده بود. هماهنگی چندرشته‌ای بین تیم‌های زنان، نورولوژی و بیهوشی از اوایل ایجاد شد. یک سزارین برنامه‌ریزی شده تحت بیهوشی نخاعی انجام شد تا خستگی تنفسی مادر و بحران مایستنی کاهش یابد. هم مادر و هم نوزاد نتایج مطلوبی داشتند و هیچ تشدیدی پس از زایمان رخ نداد. نتیجه‌گیری: نتایج مطلوب می‌تواند با تشخیص زودهنگام، نظارت و درمان مناسب با رویکرد چندرشته‌ای به دست آید. تصمیمات زایمان مبتنی بر ارزیابی تنفسی و ثبات کلی بیماری برای پیشگیری از بحران و تضمین ایمنی هم مادر و هم نوزاد حیاتی است.
