چکیده اصلی

Falls are a leading cause of injury and death among older adults, yet a persistent gap exists between available evidence-based prevention strategies and their real-world uptake. Social determinants of health shape fall risk as well as the accessibility and effectiveness of prevention interventions. Cohort evidence demonstrates that disparities in outcomes are intersectional and emerge at the community level, challenging current risk assessment tools and one-size-fits-all prevention approaches. Closing this gap requires further research, validated equity-informed assessment tools, and interventions matched to patients' social circumstances.
