چکیده اصلی

The objective of this study was to assess the trends in iron indices among non-dialysis chronic kidney disease (ND-CKD) patients with anemia attending a tertiary hospital in northern Bangladesh. This cross-sectional study was conducted on patients with ND-CKD and concurrent anemia, over a period of six months in the Department of Medicine at Rangpur Medical College Hospital, Rangpur, Bangladesh. A total of 50 patients were selected through purposive sampling. The staging of CKD and the severity of anemia were determined according to the criteria established by the National Kidney Foundation and the World Health Organization. Data collection was facilitated using a pre-structured questionnaire, and statistical analysis was carried out using SPSS version 25.0. Mean age of the patients was 47 years, with slight male predominance. Overall low iron indices were observed in 22% of the patients where 16% had absolute iron deficiency and 6% had functional deficiency. Median serum ferritin was 193 (616) ng/ml. Mean levels of serum iron, total iron binding capacity (TIBC) and transferrin saturation (TSAT) were 80.80±34.40μg/dl, 266.84±71.62μg/dl and 30.54±10.36%. Serum ferritin & TIBC tended to increase and iron and TSAT tended to decrease with the progression of CKD. But no statistical significance was noted (p>.05). Iron deficiency is prevalent in ND-CKD patients with anemia in Northern Bangladesh, indicating a need for regular monitoring and tailored management. Early intervention is crucial to improve patient outcomes and prevent severe anemia.

این مطالعه مقطعی ۵۰ بیمار مبتلا به نارسایی مزمن کلیه (ND-CKD) و کم‌خونی را در شمال بنگلادش بررسی کرد. نتایج نشان داد که ۲۲٪ از بیماران شاخص‌های آهن پایینی داشتند، از جمله ۱۶٪ کمبود مطلق آهن. با پیشرفت بیماری، فریتین و TIBC افزایش و آهن و TSAT کاهش یافتند، اما این تغییرات معناداری آماری نداشتند.

هدف این مطالعه ارزیابی روند شاخص‌های آهن در بیماران مبتلا به کم‌خونی نارسایی مزمن کلیه (ND-CKD) بدون دیالیز بود که در یک بیمارستان تخصصی شمال بنگلادش مراجعه می‌کردند. این مطالعه مقطعی در بخش پزشکی بیمارستان کالج پزشکی رانگپور، بنگلادش در طول شش ماه بر روی بیماران ND-CKD همراه با کم‌خونی انجام شد. کل ۵۰ بیمار از طریق نمونه‌گیری هدفمند انتخاب شدند. مراحل CKD و شدت کم‌خونی بر اساس معیارهای بنیاد ملی کلیه و سازمان بهداشت جهانی تعیین شدند. داده‌ها با استفاده از پرسشنامه‌های پیش‌ساخته جمع‌آوری شد و تحلیل آماری با SPSS نسخه ۲۵.۰ انجام شد. میانگین سنی بیماران ۴۷ سال بود و کمی غلبه مردان مشاهده شد. در مجموع ۲۲٪ بیماران شاخص‌های آهن پایینی داشتند که ۱۶٪ آن‌ها کمبود مطلق آهن و ۶٪ کمبود عملکردی بودند. میانه سرم فریتین ۱۹۳ (۶۱۶) ng/ml بود. میانگین سطوح سرم آهن، ظرفیت متصل‌سازی کل آهن (TIBC) و اشباع ترانسفروین ۸۰.۸۰±۳۴.۴۰ μg/dl، ۲۶۶.۸۴±۷۱.۶۲ μg/dl و ۳۰.۵۴±۱۰.۳۶٪ بود. سرم فریتین و TIBC تمایل به افزایش و آهن و TSAT تمایل به کاهش با پیشرفت CKD داشتند. اما هیچ معناداری آماری مشاهده نشد (p>.05). کمبود آهن در بیماران ND-CKD مبتلا به کم‌خونی در شمال بنگلادش شایع است که نشان‌دهنده نیاز به نظارت منظم و مدیریت متناسب است. مداخله زودهنگام برای بهبود نتایج بیمار و پیشگیری از کم‌خونی شدید حیاتی است.

روش پژوهش

این مطالعه مقطعی در بخش پزشکی بیمارستان کالج پزشکی رانگپور، بنگلادش در طول شش ماه انجام شد. ۵۰ بیمار از طریق نمونه‌گیری هدفمند انتخاب شدند. معیارهای مراحل CKD و شدت کم‌خونی بر اساس معیارهای بنیاد ملی کلیه و سازمان بهداشت جهانی تعیین شدند. تحلیل آماری با SPSS نسخه ۲۵.۰ انجام شد.

محدودیت‌ها

مطالعه مقطعی بود و نمونه‌گیری هدفمند انجام شد. معناداری آماری برای تغییرات شاخص‌های آهن با پیشرفت CKD مشاهده نشد.
