چکیده اصلی

This case report presents the management of chronic osteomyelitis of femur in a 6-year-old male child, emphasising the role of prophylactic stabilisation using a monolateral rail fixator. The child presented with persistent pus discharge, pain and difficulty in walking following an initial drainage procedure for thigh swelling at another hospital. Clinical, radiological and laboratory findings confirmed the diagnosis of chronic osteomyelitis. During surgery, a large portion of the posterolateral femoral cortex needed to be removed for thorough debridement. To maintain limb stability, a monolateral rail fixator was applied. The patient showed progressive healing on follow-up visits, with restoration of bony architecture and full return to function. The case demonstrates that monolateral rail fixators offer biomechanically superior and a patient-friendly alternative to conventional universal external fixators especially in children. Their use permits early weight bearing and thus better psychosocial outcomes and simplified postoperative care.

این گزارش موردی، استفاده از قاب‌بند سینگل‌ساید را در درمان عفونت استخوان‌سوزی مزمن فمور در یک کودک ۶ ساله بررسی می‌کند. این روش به عنوان جایگزینی دوستانه‌تر و بیومکانیکی بهتر نسبت به قاب‌بندهای کلاسیک پیشنهاد شده است.

این گزارش موردی، مدیریت عفونت استخوان‌سوزی مزمن فمور در یک پسر ۶ ساله را تشریح می‌کند و نقش ثبات‌بخشی پیشگیرانه با استفاده از قاب‌بند سینگل‌ساید را برجسته می‌سازد. کودک با ترشحات مزمن، درد و مشکل در راه رفتن پس از عمل تخلیه اولیه برای تورم ران در بیمارستان دیگر ظاهر شد. یافته‌های بالینی، رادیولوژیک و آزمایشگاهی تشخیص عفونت استخوان‌سوزی مزمن را تأیید کردند. در حین عمل، بخش بزرگی از کورتیکس پسترولateral فمور برای تخلیه کامل باید حذف می‌شد. برای حفظ ثبات اندام، قاب‌بند سینگل‌ساید اعمال شد. بیمار در ویزیت‌های پیگیری بهبود پیشرونده نشان داد و معماری استخوانی بازگشت یافت و عملکرد به طور کامل بازیابی شد. این مورد نشان می‌دهد که قاب‌بندهای سینگل‌ساید، به دلیل ویژگی‌های بیومکانیکی برتر و دوستانه بودن برای بیمار، جایگزین مناسبی برای قاب‌بندهای خارجی کلاسیک به ویژه در کودکان هستند. استفاده از آن‌ها امکان تحمل وزن زودهنگام را فراهم می‌کند که منجر به نتایج روان‌اجتماعی بهتر و مراقبت پس از عمل ساده‌تر می‌شود.
