چکیده اصلی

AIMS: This study aimed to explore dietitians' views, experiences, and perceived barriers and enablers to working with people with mental illness in Australian private and public mental health settings, with a focus on clarifying the contribution of private practice dietitians to mental healthcare. METHODS: A combination of convenience and snowball sampling was used to recruit dietitians who primarily work in private practice or public mental health sectors. Virtual semi-structured interviews were conducted in two discrete 3-6-month blocks between 2021 and 2024. A constructivist-interpretivist approach informed the development of the interview guide, data collection and analysis processes. Semi-structured interviews explored experiences, facilitators and barriers of private practice dietitians working with people with mental illness. Data were analysed by three authors using reflexive thematic analysis using semantic, inductive coding. RESULTS: In total, 14 dietitians completed interviews, comprising 7 working in private practice settings and 7 in public mental health services. Data adequacy was reached with interviews generating 5 main themes and 15 subthemes. Themes included (1) the complex needs of people living with mental illness, (2) access barriers and enablers to private practice dietitian services, (3) gaps in mental health-specific training and support for dietetics, (4) structural limitations of the private healthcare systems and (5) advantages of integrated care in specialised mental healthcare settings for working with people with mental illness. CONCLUSIONS: To harness the full potential of private practice dietitians in mental healthcare requires targeted reforms to funding, training and referral systems to ensure equitable, community-based nutrition support.

این مطالعه کیفی ۱۴ رژیم‌غذایی‌دان (۷ خصوصی و ۷ عمومی) را بررسی کرد تا تجربیات و موانع کار با افراد دارای بیماری روانی را شناسایی کند. یافته‌ها نشان داد که نیازهای پیچیده بیماران، محدودیت‌های ساختاری سیستم‌های مراقبت و کمبود آموزش تخصصی از مهم‌ترین چالش‌ها هستند. نویسندگان پیشنهاد می‌کنند که اصلاحات در بودجه، آموزش و ارجاع برای بهره‌برداری بهتر از رژیم‌غذایی‌دان‌های خصوصی در مراقبت‌های روانی ضروری است.

این مطالعه به بررسی دیدگاه‌ها، تجربیات، موانع و تسهیل‌گرانِ رژیم‌غذایی‌دان‌ها در کار با افراد دارای بیماری روانی در محیط‌های خصوصی و عمومی سلامت روان استرالیا، با تمرکز بر روشن کردن نقش رژیم‌غذایی‌دان‌های خصوصی در مراقبت‌های روانی پرداخت. از ترکیب نمونه‌گیری آسان و نمونه‌گیری برفی برای جذب رژیم‌غذایی‌دان‌هایی که عمدتاً در بخش خصوصی یا خدمات سلامت روان عمومی کار می‌کردند استفاده شد. مصاحبه‌های نیمه‌ساختاریافته مجازی در دو بازه زمانی ۳ تا ۶ ماهه بین ۲۰۲۱ و ۲۰۲۴ انجام شد. یک رویکرد ساختارگرایی-تفسیرگرایی در توسعه راهنمای مصاحبه، گردآوری داده‌ها و فرایندهای تحلیل نقش داشت. مصاحبه‌ها تجربیات، تسهیل‌گران و موانع رژیم‌غذایی‌دان‌های خصوصی را در کار با افراد دارای بیماری روانی بررسی کردند. داده‌ها توسط سه نویسنده با استفاده از تحلیل موضوعی بازتابی و کدگذاری معنایی-استنتاجی تحلیل شدند. در مجموع ۱۴ رژیم‌غذایی‌دان مصاحبه را تکمیل کردند که ۷ نفر در محیط‌های خصوصی و ۷ نفر در خدمات سلامت روان عمومی فعالیت می‌کردند. با تولید ۵ موضوع اصلی و ۱۵ زیرموضوع، داده‌ها کافی تلقی شدند. موضوعات شامل (۱) نیازهای پیچیده افراد دارای بیماری روانی، (۲) موانع و تسهیل‌گران دسترسی به خدمات رژیم‌غذایی‌دان‌های خصوصی، (۳) شکاف‌ها در آموزش و حمایت تخصصی سلامت روان برای رژیم‌غذایی، (۴) محدودیت‌های ساختاری سیستم‌های مراقبت خصوصی و (۵) مزایای مراقبت یکپارچه در محیط‌های تخصصی سلامت روان برای کار با افراد دارای بیماری روانی بود. برای بهره‌برداری از پتانسیل کامل رژیم‌غذایی‌دان‌های خصوصی در مراقبت‌های روانی، اصلاحات هدفمند در بودجه، آموزش و سیستم‌های ارجاع برای تضمین حمایت غذایی مبتنی بر جامعه و عادلانه مورد نیاز است.

روش پژوهش

این مطالعه کیفی با استفاده از رویکرد ساختارگرایی-تفسیرگرایی و تحلیل موضوعی بازتابی انجام شد. مصاحبه‌های نیمه‌ساختاریافته مجازی با ۱۴ رژیم‌غذایی‌دان (۷ خصوصی و ۷ عمومی) در بازه زمانی ۲۰۲۱ تا ۲۰۲۴ برگزار گردید.

محدودیت‌ها

محدودیت‌های این مطالعه شامل استفاده از نمونه‌گیری آسان و برفی است که ممکن است نمایندگی ناقصی از کل جمعیت رژیم‌غذایی‌دان‌ها داشته باشد. همچنین، داده‌ها صرفاً بر تجربیات خود رژیم‌غذایی‌دان‌ها متمرکز بودند.
