چکیده اصلی

BACKGROUND: With the continuous improvement of global cancer survival rates, the long-term quality of life among cancer survivors has attracted increasing attention. Glioma is a common primary malignant tumor of the central nervous system. Both the disease itself and anti-tumor therapies induce a variety of dynamically fluctuating symptoms throughout the disease course, which impose persistent physical and psychological impairments and substantially compromise patients' quality of life. Current qualitative studies have only preliminarily explored symptom-related issues in patients with glioma, lacking longitudinal analyses regarding the dynamic evolution of patients' symptom perceptions, coping strategies, and care demands. Accordingly, this study adopts a longitudinal phenomenological approach to address the gaps in existing evidence. AIMS: This longitudinal qualitative study aims to conduct a phenomenological exploration of the symptom trajectories in glioma survivorship, specifically focusing on the evolving processes of symptom recognition, the diversity of long-term coping strategies, the clinical challenges in symptom modulation, and the resultant needs for a structured intervention framework. METHODS: Based on the phenomenological approach, semi-structured interviews were implemented among 12 glioma survivors at three time points (T1, initial diagnosis; T2, 1 month after surgery; T3, 3 months after surgery). Colaizzi's method was used to analyze interview transcripts and extract the essential themes of patients' long-term survivorship experience. RESULTS: The following four themes and associated categories were identified: (1) self-perception and interpretation of the disease, (2) differentiated coping with symptoms, (3) practical dilemmas in symptom management, (4) core needs in symptom management. CONCLUSION: The symptom journey for glioma survivors is not static but a complex trajectory marked by significant and evolving burdens. Our findings, extending from our earlier research, underscore the critical need for healthcare systems to move beyond episodic care. Future efforts must focus on developing longitudinal, patient-centered intervention frameworks that address the unmet needs across the survivorship continuum, thereby effectively reducing symptom burden and enhancing quality of life.

با بهبود مداوم نرخ بقا سرطان در سطح جهان، کیفیت زندگی بلندمدت بازماندگان سرطان توجه بیشتری را به خود جلب کرده است. گلیوم یک تومور بدخیم اولیه شایع در سیستم عصبی مرکزی است. هم خود بیماری و هم درمان‌های ضد تومور طی مسیر بیماری طیف وسیعی از علائم پویا و متغیر ایجاد می‌کنند که بارهای فیزیکی و روانی ماندگاری ایجاد کرده و کیفیت زندگی بیماران را به‌طور قابل توجهی کاهش می‌دهند. مطالعات کیفی موجود تنها به طور مقدماتی مسائل مرتبط با علائم در بیماران گلیوم را بررسی کرده‌اند و تحلیل‌های طولی در مورد تکامل پویای درک بیماران از علائم، استراتژی‌های مقابله‌ای و نیازهای مراقبتی را ندارند. این مطالعه با رویکرد فرا-تجربی طولی، شکاف‌های موجود در شواهد را پر می‌کند. هدف این مطالعه کیفی طولی، بررسی فرا-تجربی مسیرهای علائم در بقا گلیوم است که به طور خاص بر فرآیندهای در حال تغییر شناسایی علائم، تنوع استراتژی‌های مقابله‌ای بلندمدت، چالش‌های بالینی در تنظیم علائم و نیازهای ناشی از آن‌ها برای یک چارچوب مداخله‌ای ساختاریافته تمرکز دارد. بر اساس رویکرد فرا-تجربی، مصاحبه‌های نیمه‌ساختاریافته در ۱۲ بازماندگان گلیوم در سه نقطه زمانی (T1، تشخیص اولیه؛ T2، یک ماه پس از جراحی؛ T3، سه ماه پس از جراحی) انجام شد. روش کولایزی برای تحلیل ترانکریپت‌های مصاحبه و استخراج موضوعات اصلی تجربه بقا بلندمدت بیماران استفاده شد. چهار موضوع و دسته‌بندی‌های مرتبط شناسایی شدند: (۱) خودبینی و تفسیر بیماری، (۲) مقابله‌ای متفاوت با علائم، (۳) دغدغه‌های عملی در مدیریت علائم، (۴) نیازهای اصلی در مدیریت علائم. مسیر علائم بازماندگان گلیوم پویا نیست، بلکه یک مسیر پیچیده با بارهای قابل توجه و در حال تغییر است. یافته‌های ما، که از تحقیقات قبلی ما گسترش می‌یابد، بر نیاز حیاتی سیستم‌های مراقبتی برای فراتر رفتن از مراقبت‌های پراکنده تأکید می‌کند. تلاش‌های آینده باید بر توسعه چارچوب‌های مداخله‌ای طولی و متمرکز بر بیمار تمرکز کند که نیازهای برآورده نشده در طیف بقا را پوشش دهد و در نتیجه بار علائم را به‌طور مؤثر کاهش و کیفیت زندگی را ارتقا دهد.

روش پژوهش

این مطالعه کیفی با رویکرد فرا-تجربی طولی انجام شد. مصاحبه‌های نیمه‌ساختاریافته در ۱۲ بازماندگان گلیوم در سه نقطه زمانی (T1، T2، T3) انجام شد. تحلیل داده‌ها با استفاده از روش کولایزی انجام شد.

محدودیت‌ها
