چکیده اصلی

BACKGROUND: Cognitive impairment in older adults is associated with substantial functional decline and care burden. The ketogenic diet (KD) may improve cognition, but evidence in older adults with cognitive impairment remains inconsistent. This study evaluated the effects of KD, including related ketogenic dietary strategies, on cognitive outcomes and metabolic markers. METHODS: PubMed, Web of Science, Embase, the Cochrane Library, CNKI, Wanfang, and VIP were searched from inception to July 2025 for randomized controlled trials of KD or related ketogenic dietary strategies in older adults with cognitive impairment. Two reviewers independently screened studies, extracted data, and assessed risk of bias. Meta-analysis was performed using Review Manager 5.4. RESULTS: Twelve randomized controlled trials involving 707 participants were included. Compared with control interventions, KD-related dietary strategies improved overall cognitive function [SMD = 0.34, 95% CI (0.16 to 0.52), p < 0.01]. Domain-specific analyses showed significant improvements in memory [SMD = 0.53, 95% CI (0.10 to 0.96), p = 0.01] and language function [SMD = 0.43, 95% CI (0.16 to 0.70), p = 0.002], whereas no significant effects were observed for attention or executive function. KD-related strategies increased acetoacetate and β-hydroxybutyrate concentrations, indicating effective ketone production. Regarding metabolic safety, KD-related interventions increased total cholesterol and LDL-C, while triglycerides and HDL-C remained unchanged. CONCLUSIONS: KD and related ketogenic dietary strategies may improve cognitive performance and enhance ketone body production in older adults with mild cognitive impairment or Alzheimer's disease. The potential metabolic implications associated with lipid changes should be considered alongside cognitive benefits.

Britanin improved high-fat diet-induced obesity by regulating the MAPK signaling pathway.

BACKGROUND: The escalating prevalence of obesity has made it a critical public health concern. There is an urgent need to identify naturally derived compounds with anti-obesity potential. Britanin (BRI), a bioactive sesquiterpene lactone derived from Inula species, has shown promise in metabolic disorder management, but its anti-obesity mechanisms remain uncharacterized. OBJECTIVES: Combining animal experiments and network pharmacology…

Interleukin-17A Links Obesity-Induced Inflammation to Behavioral Disorders and Autonomic Imbalance.

Obesity is increasingly recognized as a chronic inflammatory condition, often driven by prolonged high-fat diet (HFD) consumption. Beyond causing metabolic dysregulation, obesity can disrupt immune-brain communication, contributing to the development of emotional disorders. During the early stages of HFD exposure, proinflammatory cytokines such as interleukin-1β (IL-1β), Tumor necrosis factor-alpha (TNF-α), and IL-6 are markedly elevat…

Refining Genetic Instruments for Dietary Intake Mendelian Randomization Using Phenome-Wide Association Studies.

Most Mendelian randomization (MR) of dietary intake use the full set of genome-wide significant (GWS) variants in the instrumental variable (IV), likely biasing causal estimates due to pleiotropy. To characterize the common methods to handle pleiotropy in dietary intake MR, we conducted a scoping review of the literature on dietary intake MR studies. We extracted information on IV construction, assessment of pleiotropy, and sensitivity…

Choosing Not to Eat Meat: What Characterizes Independently Vegetarian Children?

A growing body of research has aimed to map the characteristics of extraordinary altruists-such as those who have donated a kidney to a stranger. However, this research has primarily focused on adults, and we know little about the development of extraordinary altruism. While children cannot donate their organs to strangers nor large portions of their income to charity, they can choose not to eat animals. In this study we examine indepe…
