چکیده اصلی

Drug overdose deaths in the United States reached record levels during the unregulated ("illicit") fentanyl era before recently declining. A plausible hypothesis is that a sudden drop in fentanyl purity ("supply shock") beginning in 2023 caused the downturn. We evaluated this hypothesis by replicating a published analysis with regional overdose data, using transfer function models accounting for time trends and autocorrelation; negative controls tested for spurious correlation. Replicating the original paper, we extracted fentanyl purity data from a Drug Enforcement Administration (DEA) figure; overdose mortality used standard vital statistics sources. We found rising national fentanyl purity did not track fatal fentanyl overdose rates in most United States regions, showing only modest association in the West. When both purity and mortality later declined, the observed associations were also seen with unrelated macroeconomic indicators that shared the same time pattern, raising concerns about spurious correlation in the original study. Canadian analyses also revealed variation in both strength and direction of association between fentanyl purity and overdose deaths at subprovince level. Differential selection bias in law enforcement purity assessment may have compromised longitudinal and geographic generalizability of DEA data, notably stimulated reporting for higher-purity wholesale samples in 2023-2024. National fentanyl purity alone does not provide sufficient explanation for the recent decline in overdose deaths. Reductions in localized purity may influence localized overdose decreases, but alternative causes must be considered nationally, including drug supply changes, public health interventions, demographic shifts, behavior changes, post-pandemic recovery, cartel dynamics, and fewer initiates to illicit opioid use.

Associations Between Pain Severity and Opioid Agonist Treatment Among People With Drug Injection Experience in Australia.

INTRODUCTION: Pain is highly prevalent among people with opioid use disorder (OUD), yet it is often undertreated. Whether opioid agonist treatment (OAT) improves pain outcomes is unknown among this target population. This research aimed to explore associations between pain severity and current OAT status in a longitudinal cohort of people who inject drugs. METHODS: Data were drawn from the Melbourne Injecting Drug User Cohort Study (Su…

Clinical effectiveness and safety of metadoxine in the management of acute alcohol intoxication: A single-center retrospective cohort study.

BACKGROUND: Acute alcohol intoxication (AAI) is a common emergency with no specific antidote. Metadoxine has shown potential but lacks sufficient real-world evidence, particularly in Chinese populations. OBJECTIVES: To evaluate the clinical efficacy and safety of metadoxine in patients with acute alcohol intoxication. METHODS: This single-center retrospective cohort study included 124 patients with AAI admitted to an emergency departme…

Environmental Context and Reporting Completeness of Event-Based Ecological Momentary Assessment in Cigarette and Cannabis Use.

INTRODUCTION: Ecological momentary assessment (EMA) allows researchers to capture substance use as it occurs in daily life, but participants may not report every use event. Little is known about whether the environments where people spend time affect the completeness of these reports. METHODS: We examined how consistently young adults reported cigarette and cannabis use through event-based EMA and whether reporting differed by the soci…

The Impact of Heterogeneity in Individual Characteristics and Motivation to Quit Heroin Use on Contingency Management and Opioid Agonist Treatment Response: A Latent Class Analysis.

INTRODUCTION: Contingency Management (CM) involves the systematic application of positive reinforcement (financial incentives) to promote behaviour change, to improve treatment outcomes for individuals with opioid use disorder (OUD). To better understand patient heterogeneity, this study sought to identify meaningful subgroups/classes underlying the population based on characteristic groupings, followed by determining whether CM effect…
