چکیده اصلی

PURPOSE: Achondroplasia is a genetic syndrome characterized by short stature and rhizomelic shortening of the limbs. Many patients also exhibit craniocervical junction stenosis, which often progresses to spinal cord compression or hydrocephalus. Foramen magnum decompression (FMD) is performed to relieve stenosis at the craniovertebral junction. The purpose of this study was to analyze the clinical outcomes of FMD in achondroplasia patients and to discuss restenosis due to bone regrowth (bony restenosis), an important cause of postoperative worsening. METHODS: Medical records and brain imaging data of 89 pediatric achondroplasia patients who visited our institution between January 2012 and April 2022 were retrospectively analyzed. RESULTS: Among 89 achondroplasia patients, FMD was performed in 38 patients. During the median 60 months of follow-up after FMD, eleven patients developed warning changes at MRI. Warning changes consisted of four types: restenosis due to bone regrowth (bony restenosis), restenosis due to soft tissue thickening, de novo T2 HSI of the cervical spinal cord, and progression of ventriculomegaly. Patients with bony restenosis (3 of 38; 7.9%) required reoperation of FMD. The age of restenosis was 46, 48, and 98 months old, and the time interval after FMD was 38, 34, and 87 months, respectively. Removal of bony spur was performed and no recurrence of bony restenosis was found until the most recent follow-up. CONCLUSION: Bony restenosis was observed in 3 of 38 patients (7.9%) after FMD during the follow-up. Regular imaging studies and close observation until late childhood may be reasonable for early detection and management of bony restenosis.

این مطالعه به تحلیل نتایج بالینی و بروز بازگشتی استخوانی پس از باز کردن منفذ جمجمه (FMD) در بیماران مولد آکندروپلازی پرداخته است. در میان ۳۸ بیمار تحت عمل، بازگشتی استخوانی در ۳ مورد (۷.۹٪) رخ داد که نیازمند عمل مجدد بود. مطالعات تصویربرداری منظم در دوران کودکی می‌تواند به تشخیص زودهنگام این عارضه کمک کند.

هدف: آکندروپلازی یک سندرم ژنتیکی با قد کوتاه و کوتاهی رادیال اندام‌هاست. بسیاری از بیماران نیز تنگی اتصال جمجمه‌ای-مخچه‌ای را نشان می‌دهند که اغلب به فشار بر نخاع یا آب‌مروارید منجر می‌شود. باز کردن منفذ جمجمه (FMD) برای کاهش تنگی در اتصال جمجمه‌ای-مخچه‌ای انجام می‌شود. هدف این مطالعه تحلیل نتایج بالینی FMD در بیماران آکندروپلازی و بحث در مورد بازگشتی استخوانی ناشی از رشد مجدد استخوان، یک علت مهم بدتر شدن پس از عمل. روش‌ها: سوابق پزشکی و داده‌های تصویربرداری مغزی ۸۹ بیمار مولد آکندروپلازی که بین ژانویه ۲۰۱۲ و آوریل ۲۰۲۲ به موسسه ما مراجعه کردند، به صورت بازگشتی تحلیل شدند. نتایج: از ۸۹ بیمار آکندروپلازی، FMD در ۳۸ بیمار انجام شد. در طول پیگیری میانگین ۶۰ ماهه پس از FMD، ۱۱ بیمار تغییرات هشداردهنده در MRI نشان دادند. تغییرات هشداردهنده شامل چهار نوع بود: بازگشتی استخوانی، بازگشتی ناشی از ضخیم‌شدن بافت نرم، HSI T2 جدید نخاع گردن و پیشرفت آب‌مروارید. بیماران با بازگشتی استخوانی (۳ از ۳۸؛ ۷.۹٪) نیاز به عمل مجدد FMD داشتند. سن بازگشتی ۴۶، ۴۸ و ۹۸ ماه و فاصله زمانی پس از FMD به ترتیب ۳۸، ۳۴ و ۸۷ ماه بود. برداشتن تیغه استخوانی انجام شد و تا پیگیری آخرین، بازگشتی استخوانی تکرار نشد. نتیجه‌گیری: بازگشتی استخوانی در ۳ از ۳۸ بیمار (۷.۹٪) پس از FMD در طول پیگیری مشاهده شد. مطالعات تصویربرداری منظم و مشاهده نزدیک تا دوران کودکی ممکن است برای تشخیص و مدیریت زودهنگام بازگشتی استخوانی منطقی باشد.

روش پژوهش

این مطالعه به صورت بازگشتی بر روی سوابق پزشکی و داده‌های تصویربرداری مغزی ۸۹ بیمار مولد آکندروپلازی انجام شد که بین ژانویه ۲۰۱۲ و آوریل ۲۰۲۲ مراجعه کرده بودند.
