چکیده اصلی

OBJECTIVES: This study aimed to explore and synthesise interpreters' perspectives on the provision of sexual and reproductive health (SRH) care to Eritrean and Somali migrant women in Switzerland, and to identify and analyse the professional and ethical challenges they encounter in this context. DESIGN: An exploratory qualitative study from a social constructionist perspective. The interviews were recorded, transcribed, categorised and analysed through thematic analysis. SETTING AND PARTICIPANTS: The study was conducted in the canton of Basel-Stadt, northwestern Switzerland. We have purposively selected 10 Eritrean and Somali intercultural interpreters serving as crucial intermediaries in assisting immigrant women to access SRH services. RESULTS: Our analysis of the interpreters' perspectives revealed three key thematic areas that they identified as central challenges for their Eritrean and Somali immigrant women: (1) perceived limited health literacy among patients, (2) sociocultural barriers that interpreters observed hindering patient provider communication and (3) structural difficulties that interpreters reported their patients faced in navigating the Swiss healthcare system. These challenges have now persisted for a long time, and research findings and recommendations do not appear to have changed practices. For example, while abortion services are fully covered by Swiss health insurance, contraceptive methods often require out-of-pocket payments. This financial disparity may discourage appropriate use of SRH services among immigrant women. The lack of culturally competent care and the limited availability of funding for intercultural interpreters were among important bottlenecks identified. CONCLUSIONS: This study explored interpreters' perspectives on the provision of SRH care to Eritrean and Somali immigrant women in Switzerland. Despite the outstanding Swiss healthcare system, a majority of Eritrean and Somali refugee women struggle to benefit from SRH services due to limited health literacy, language barriers and challenges in navigating the Swiss healthcare system. Partnerships and integration of community entities, such as the Swisso-Kalmo association for Somali women and different Eritrean communities, could contribute via facilitating entry points for access to SRH healthcare.

هدف این مطالعه بررسی و ترکیب دیدگاه‌های مترجمان در مورد ارائه مراقبت‌های سلامت باروری و تولیدمثل (SRH) به زنان مهاجر اریتره‌ای و سومالی در سوئیس و شناسایی و تحلیل چالش‌های حرفه‌ای و اخلاقی که در این زمینه با آن‌ها روبرو هستند. این مطالعه، یک بررسی کیفی اکتشافی از دیدگاه ساختارگرای اجتماعی است. مصاحبه‌ها ضبط، ترجمه، دسته‌بندی و تحلیل موضوعی شده‌اند. این مطالعه در کانتون باسل-اشتات، غرب سوئیس انجام شد. ۱۰ مترجم بین‌فرهنگی اریتره‌ای و سومالی که نقش واسطه‌های حیاتی در کمک به زنان مهاجر برای دسترسی به خدمات SRH را داشتند، به صورت هدفمند انتخاب شدند. تحلیل دیدگاه‌های مترجمان سه محور موضوعی کلیدی را آشکار کرد که آن‌ها به عنوان چالش‌های اصلی برای زنان مهاجر اریتره‌ای و سومالی خود می‌دانستند: (۱) باور به سواد سلامت محدود در بیماران، (۲) موانع اجتماعی-فرهنگی که مترجمان مشاهده می‌کردند مانع ارتباط بیمار و ارائه‌دهنده می‌شوند و (۳) دشواری‌های ساختاری که مترجمان گزارش کردند بیمارانشان در هدایت سیستم مراقبت سوئیس با آن‌ها روبرو هستند. این چالش‌ها برای مدت طولانی ادامه داشته‌اند و یافته‌های تحقیق و توصیه‌ها به نظر می‌رسد تغییراتی در عمل ایجاد نکرده‌اند. به عنوان مثال، در حالی که خدمات سقط جنین کاملاً توسط بیمه سوئیس پوشش داده می‌شود، روش‌های ضدبارداری اغلب نیاز به پرداخت نقدی دارند. این تفاوت مالی ممکن است استفاده مناسب از خدمات SRH را در زنان مهاجر کاهش دهد. عدم وجود مراقبت با سواد فرهنگی و محدودیت بودجه برای مترجمان بین‌فرهنگی از جمله شلوغی‌های مهم شناسایی شده‌اند. این مطالعه دیدگاه‌های مترجمان را در مورد ارائه مراقبت‌های SRH به زنان مهاجر اریتره‌ای و سومالی در سوئیس بررسی کرد. با وجود سیستم مراقبت سوئیس برجسته، اکثر زنان پناهنده اریتره‌ای و سومالی برای بهره‌مندی از خدمات SRH با محدودیت سواد سلامت، موانع زبانی و چالش‌های هدایت سیستم مراقبت سوئیس دست‌وپنج نرم می‌کنند. همکاری و ادغام نهادهای جامعه، مانند انجمن سوئیس-کالمو برای زنان سومالی و جوامع مختلف اریتره‌ای، می‌تواند از طریق تسهیل نقاط ورود برای دسترسی به مراقبت‌های SRH کمک کند.
