چکیده اصلی

Candidozyma auris is a multidrug-resistant yeast that can cause invasive infections and healthcare-associated outbreaks. This retrospective study evaluated seven pediatric patients in whom C. auris was detected in a tertiary pediatric intensive care unit in Türkiye between February 2023 and February 2024. The mean age of the patients was 95.71 ± 80.52 months. Five patients had chronic illnesses and a history of multiple hospitalizations. C. auris was isolated from blood (five isolates), urine (three isolates), skin swab (three isolates), and catheter tip (one isolate) cultures. At initial evaluation, one patient was classified as colonized, whereas six were diagnosed with clinically significant C. auris infections, including bloodstream infection, catheter-related bloodstream infection, and urinary tract infection. Of the five patients, three were initially diagnosed with urinary tract infection; however, one of these patients was ultimately considered to have urinary colonization rather than infection. Six patients received antifungal therapy. Five were treated for candidemia, and two of these patients also had concurrent urinary tract infection. All isolates were susceptible to echinocandins. Two patients died due to C. auris-related candidemia. Herein, we present our experience in controlling a suspected healthcare-associated C. auris outbreak and the treatment approaches implemented following the first identified C. auris case in our hospital.

در این مطالعه بازنگری، ۷ بیمار کودک در یک مرکز tertiary در ترکیه با کاندیدا آوریس مواجه شدند. اکثر بیماران دارای بیماری‌های مزمن و سابقه بستری‌های متعدد بودند. عفونت‌ها شامل کاندیدمی و عفونت ادراری بود. تمام ایزوله‌ها به اپی‌کندین‌ها حساس بودند و دو بیمار درگذشتند.

کاندیدا آوریس یک قارچ مقاوم به داروهای متعدد است که می‌تواند عفونت‌های تهاجمی و شیوع‌های بیمارستانی را ایجاد کند. این مطالعه بازنگری ۷ بیمار کودک را ارزیابی کرد که در آن کاندیدا آوریس در یک واحد مراقبت‌های ویژه کودکان در ترکیه بین فوریه ۲۰۲۳ و فوریه ۲۰۲۴ شناسایی شد. میانگین سن بیماران ۹۵.۷۱ ± ۸۰.۵۲ ماه بود. پنج بیمار دارای بیماری‌های مزمن و سابقه بستری‌های متعدد بودند. کاندیدا آوریس از کشت خون (۵ ایزوله)، ادرار (۳ ایزوله)، کشت پوست (۳ ایزوله) و نوک سوند (۱ ایزوله) جدا شد. در ارزیابی اولیه، یک بیمار کولونیزه و شش بیمار دارای عفونت بالینی مهم کاندیدا آوریس تشخیص داده شدند. پنج بیمار ابتدا عفونت ادراری تشخیص داده شدند، اما یکی از این بیماران در نهایت به عنوان کولونیزه ادراری در نظر گرفته شد. شش بیمار درمان ضدقارچ دریافت کردند. پنج نفر برای کاندیدمی درمان شدند و دو نفر از این بیماران عفونت ادراری همزمان داشتند. تمام ایزوله‌ها به اپی‌کندین‌ها حساس بودند. دو بیمار به دلیل کاندیدمی مرتبط با کاندیدا آوریس درگذشتند.

روش پژوهش

این مطالعه بازنگری بود و داده‌ها از یک مرکز tertiary در ترکیه بین فوریه ۲۰۲۳ و فوریه ۲۰۲۴ جمع‌آوری شد.
