چکیده اصلی

PURPOSE: We conducted an analysis to identify preoperative predictors of vesicoureteral reflux (VUR) correction following endoscopic Deflux injection (EDI). METHODS: We retrospectively reviewed 109 patients with primary VUR (173 renal units) aged < 16 years who underwent EDI between 2016 and 2025. If grade II-V VUR persisted on postoperative voiding cystoureterography, injections were repeated up to two additional times. Preoperative variables included constipation, renal scarring, and VUR grade. RESULTS: VUR was corrected in 86 units (49.7%) after the first injection and in 152 (87.9%) after up to three injections. Univariable analysis showed that high-grade VUR, renal scarring, and constipation were associated with failure after the first injection, whereas only constipation was associated with failure after up to three injections. Multivariable analysis identified high-grade VUR, renal scarring and constipation as independent risk factors for failure of the first injection. These associations persisted even after exclusion of clinically evident bladder and bowel dysfunction (BBD). CONCLUSIONS: Constipation was an independent risk factor for failure after the first injection and remained associated with failure after repeated injections, even after exclusion of clinically evident BBD. It may serve as a clinically relevant and easily assessable predictor, particularly in young children in whom bladder function is difficult to assess.

هدف: شناسایی عوامل پیش‌بینی‌کننده قبل از عمل برای اصلاح بازگشت ادراری ویسکو-اورترال (VUR) پس از تزریق دفلکس درون‌کششی (EDI). روش‌ها: بررسی پس‌رونده ۱۰۹ بیمار مبتلا به VUR اولیه (۱۷۳ واحد کلیوی) زیر ۱۶ سال که در سال‌های ۲۰۱۶ تا ۲۰۲۵ تحت EDI قرار گرفتند. اگر در سونوگرافی ادرار-مجاری ادراری پس از عمل، VUR درجه II تا V باقی می‌ماند، تزریق‌ها تا دو بار دیگر تکرار شدند. متغیرهای قبل از عمل شامل یبوست، اسکار کلیوی و درجه VUR بود. نتایج: VUR در ۸۶ واحد (۴۹.۷٪) پس از تزریق اول و در ۱۵۲ (۸۷.۹٪) پس از حداکثر سه تزریق اصلاح شد. تحلیل تک‌متغیره نشان داد که VUR درجه بالا، اسکار کلیوی و یبوست با شکست پس از تزریق اول مرتبط بودند، در حالی که فقط یبوست با شکست پس از حداکثر سه تزریق مرتبط بود. تحلیل چندمتغیره یبوست، اسکار کلیوی و VUR درجه بالا را به عنوان عوامل خطر مستقل برای شکست تزریق اول شناسایی کرد. این روابط حتی پس از حذف اختلالات بالینی مثبت مثانه و روده (BBD) باقی ماند. نتیجه‌گیری: یبوست یک عامل خطر مستقل برای شکست پس از تزریق اول بود و با شکست پس از تزریق‌های تکراری مرتبط ماند، حتی پس از حذف BBD بالینی. این می‌تواند به عنوان یک پیش‌بینی بالینی مرتبط و آسان ارزیابی، به ویژه در کودکان جوان که ارزیابی عملکرد مثانه دشوار است، خدمت کند.
