PURPOSE: Adults with learning disabilities experience a poorer quality of life than the general population. Social care can critically support such adults and their informal carers, but limited literature exists about occupational therapy provision. This study explored the extent to which social care occupational therapists support communication and collaboration, and make a difference. METHODS: Eight social care occupational therapists were recruited through professional networks. Online interviews were undertaken using topic guides co-designed with professionals and experts by experience. Reflexive thematic analysis was employed, informed by critical realism. FINDINGS: Therapists pursued inclusive practices with adults with learning disabilities and valued informal carers. The communication challenges increased the reliance on carer input and raised doubts about interventions reflecting the wishes of adults with learning disabilities. Carer disagreements were navigated through diplomacy and positive risk-taking, but concerns about moving and handling safety led to faltering collaboration. Therapists felt that they enhanced community engagement and reduced carer burden, but workload limited their scope of practice. CONCLUSIONS: Social care occupational therapists strive to work inclusively with adults with learning disabilities and their informal carers, with beneficial outcomes, requiring various profession-based and interpersonal skillsets. However, communication difficulties create uncertainty. Practice-improvement opportunities include additional training and enhanced multidisciplinary working.

Dose-Response Relationship Between Physical Activity and Intrinsic Capacity Decline Among Hospitalised Older Patients.

OBJECTIVE: Intrinsic capacity (IC) serves as the foundation for functional performance in older adults, and its decline exerts a significant negative impact on their overall health status. However, the dose-response relationship between physical activity (PA) and IC decline is unclear. The objective of this study was to examine the dose-response relationship between PA and IC decline in hospitalised older adults. METHODS: This cross-se…

Focused Extracorporeal Shock Wave Therapy Versus Therapeutic Ultrasound as Adjuncts to Exercise-Based Rehabilitation for Functional Limitation in Ankylosing Spondylitis: An Assessor-Blinded Randomized Comparative Trial.

BACKGROUND: The comparative effectiveness of adjunctive physical modalities alongside exercise for ankylosing spondylitis (AS) remains inadequately evaluated. OBJECTIVE: To compare the relative association of focused extracorporeal shock wave therapy (ESWT) versus therapeutic ultrasound, both added to identical exercise-based rehabilitation, with long-term functional outcomes in AS. METHODS: This assessor-blinded randomized comparative…

Ultrasound-guided high-voltage vs conventional pulsed radiofrequency in elderly cervical radiculopathy: A randomized controlled trial.

BACKGROUND: Elderly patients with cervical radiculopathy present therapeutic challenges owing to comorbidities and medication-related risks. Long-term pharmacotherapy and surgical interventions are often suboptimal, necessitating evaluation of optimized pulsed radiofrequency strategies under image guidance. OBJECTIVES: This superiority trial compared the efficacy and safety of ultrasound-guided cervical nerve root high-voltage pulsed r…
