چکیده اصلی

This study aimed to integrate in vitro dissolution testing, in vivo pharmacokinetic (PK) studies, and computational simulation to develop a physiologically based biopharmaceutics model (PBBM) for fixed-dose combination ibuprofen/diphenhydramine citrate tablets, thereby evaluating bioequivalence (BE) and establishing a dissolution safe space. Initially, a PBBM model was constructed using B2O Simulator software. Subsequently, an integrated approach was employed, combining dissolution profiles with physiological parameters to develop a virtual bioequivalence (VBE) model. Through simulations involving virtual batches exhibiting diverse dissolution characteristics, the safe dissolution range was determined. Consequently, the PBBM models for ibuprofen and diphenhydramine were successfully validated. Furthermore, the VBE model demonstrated favorable bioequivalence, with an absolute average fold error (AAFE) of less than 1.5 between predicted and observed values for key PK parameters (Cmax and AUC). Scenario analyses supported a formulation- and medium-specific dissolution criterion of no less than 85% dissolution within 30 min for both APIs in pH 6.5 phosphate buffer. Additionally, scenario analyses suggested that, within the simulated two-treatment, two-period crossover settings evaluated in this study, a sample size of approximately 40 subjects may represent a practical lower bound for obtaining stable BE conclusions. In conclusion, this study provides a product-specific, model-informed workflow for bioequivalence risk assessment of ibuprofen/diphenhydramine citrate tablets. The application of PBBM and VBE modeling offers guidance for quantitative BE risk assessment, thereby holding promise to accelerate the development process while reducing associated costs. ترجمه فارسی چکیده

این مطالعه به منظور توسعه یک مدل PBBM برای قرص‌های ترکیبی ایبوپروفن/دی‌فن‌هیدرامین سیترات و ارزیابی معادل‌سازی زیستی (BE) و تعیین فضای ایمن حل شدن، با ادغام آزمایش حل شدن در vitro، مطالعات فارماکوکینتیک در vivo و شبیه‌سازی محاسباتی انجام شد. ابتدا یک مدل PBBM با استفاده از نرم‌افزار B2O Simulator ساخته شد. سپس یک رویکرد یکپارچه با ترکیب پروفایل‌های حل شدن با پارامترهای فیزیولوژیک برای توسعه یک مدل معادل‌سازی زیستی مجازی (VBE) به کار گرفته شد. از طریق شبیه‌سازی‌های شامل دسته‌های مجازی با ویژگی‌های حل شدن متنوع، محدوده ایمن حل شدن تعیین گردید. در نهایت، مدل‌های PBBM برای ایبوپروفن و دی‌فن‌هیدرامین با موفقیت تایید شدند. مدل VBE نشان داد معادل‌سازی زیستی مطلوبی با خطای لغزنده متوسط مطلق (AAFE) کمتر از ۱.۵ بین مقادیر پیش‌بینی شده و مشاهده شده برای پارامترهای کلیدی PK (Cmax و AUC). تحلیل سناریوها از یک معیار حل شدن مخصوص فرمولاسیون و محیط برابر یا بیشتر از ۸۵٪ در ۳۰ دقیقه برای هر دو داروی فعال در فسفات pH ۶.۵ حمایت کرد. همچنین تحلیل سناریوها نشان داد که در محیط‌های دو درمانی و دو دوره متقاطع مورد ارزیابی، تعداد نمونه حدود ۴۰ شرکت‌کننده ممکن است یک حد پایین عملی برای دستیابی به نتایج معادل‌سازی زیستی پایدار باشد. این مطالعه یک جریان کار مبتنی بر مدل برای ارزیابی ریسک معادل‌سازی زیستی محصولات خاص ایبوپروفن/دی‌فن‌هیدرامین سیترات ارائه می‌دهد. کاربرد مدل‌سازی PBBM و VBE راهنمایی‌ای برای ارزیابی کمی ریسک معادل‌سازی زیستی ارائه می‌دهد که امیدوارکننده است تا فرآیند توسعه را تسریع و هزینه‌های مرتبط را کاهش دهد.
