چکیده اصلی

BACKGROUND: This study aimed to compare the electrocardiographic (ECG) depolarization and repolarization parameters among patients with cannabis use disorder (CUD), methamphetamine use disorder (MUD), and healthy controls (HC). METHODS: This retrospective cross-sectional study included 63 patients with CUD, 63 with MUD, and 63 age-matched male HC's recruited from a substance addiction treatment center. Standard 12-lead ECGs were obtained within the first hour of admission. Parameters assessed included QTc, QT dispersion (QTcd), JTc, JT dispersion (JTcd), Tp-e interval, Tp-e/QTc ratio, P-wave dispersion (PWD), frontal QRS-T angle, frontal QRS axis, heart rate, and fragmented QRS (fQRS). RESULTS: The MUD group demonstrated significantly prolonged QTc, QTcd, and JTcd values compared to both CUD and HC groups (all p < 0.001). ROC analysis identified JTcd as the most discriminative parameter for methamphetamine use disorder (AUC=0.718), outperforming QTc (AUC=0.682) and QTcd (AUC=0.676). Heart rate was significantly elevated in the MUD group compared to CUD (p = 0.008). Cannabis users showed no significant differences in repolarization parameters compared to HCs; however, fQRS prevalence was significantly elevated in both the CUD (14.3%) and MUD (17.5%) groups relative to HCs (1.6%, p = 0.011), suggesting subclinical myocardial structural abnormalities in both groups. DISCUSSION: These findings indicate that methamphetamine use is associated with substantial ventricular repolarization disturbances, with JTcd emerging as the most sensitive ECG marker. Although cannabis did not affect repolarization parameters, elevated fQRS prevalence points to potential subclinical myocardial injury. Routine ECG assessment is warranted in patients with substance use disorders.

Associations Between Pain Severity and Opioid Agonist Treatment Among People With Drug Injection Experience in Australia.

INTRODUCTION: Pain is highly prevalent among people with opioid use disorder (OUD), yet it is often undertreated. Whether opioid agonist treatment (OAT) improves pain outcomes is unknown among this target population. This research aimed to explore associations between pain severity and current OAT status in a longitudinal cohort of people who inject drugs. METHODS: Data were drawn from the Melbourne Injecting Drug User Cohort Study (Su…

Clinical effectiveness and safety of metadoxine in the management of acute alcohol intoxication: A single-center retrospective cohort study.

BACKGROUND: Acute alcohol intoxication (AAI) is a common emergency with no specific antidote. Metadoxine has shown potential but lacks sufficient real-world evidence, particularly in Chinese populations. OBJECTIVES: To evaluate the clinical efficacy and safety of metadoxine in patients with acute alcohol intoxication. METHODS: This single-center retrospective cohort study included 124 patients with AAI admitted to an emergency departme…

Environmental Context and Reporting Completeness of Event-Based Ecological Momentary Assessment in Cigarette and Cannabis Use.

INTRODUCTION: Ecological momentary assessment (EMA) allows researchers to capture substance use as it occurs in daily life, but participants may not report every use event. Little is known about whether the environments where people spend time affect the completeness of these reports. METHODS: We examined how consistently young adults reported cigarette and cannabis use through event-based EMA and whether reporting differed by the soci…

The Impact of Heterogeneity in Individual Characteristics and Motivation to Quit Heroin Use on Contingency Management and Opioid Agonist Treatment Response: A Latent Class Analysis.

INTRODUCTION: Contingency Management (CM) involves the systematic application of positive reinforcement (financial incentives) to promote behaviour change, to improve treatment outcomes for individuals with opioid use disorder (OUD). To better understand patient heterogeneity, this study sought to identify meaningful subgroups/classes underlying the population based on characteristic groupings, followed by determining whether CM effect…
