Protecting the Privacy of Patients With Impaired Consciousness in Intensive Care: A Qualitative Study.

چکیده اصلی

BACKGROUND: Maintaining privacy in intensive care units (ICUs) is challenging because of high workload, invasive procedures, continuous monitoring and shared care environments. Patients with impaired consciousness may be particularly vulnerable because their ability to express privacy preferences, assert personal boundaries and advocate for themselves is limited. AIM: To explore intensive care nurses' experiences of protecting privacy when caring for patients with impaired consciousness. STUDY DESIGN: A descriptive qualitative study was conducted with 20 intensive care nurses in Türkiye between March and April 2026. Data were collected through semi-structured online interviews and analysed using inductive thematic analysis. The study was reported in accordance with the Consolidated Criteria for Reporting Qualitative Research guidelines. FINDINGS: Four themes were identified: the marginalisation of privacy under clinical priorities; structural and process-related constraints on privacy in intensive care; loss of subjectivity and the mechanisation of care in patients with impaired consciousness; and ethical and psychosocial implications of privacy violations. CONCLUSIONS: Privacy in intensive care is shaped not only by individual ethical awareness but also by workload, environmental conditions, care routines and communication practices. Patients with impaired consciousness have the same rights to privacy and dignity as other patients; however, their reduced ability to express preferences, assert personal boundaries and advocate for themselves may make privacy violations less visible and increase their dependence on nurses for protection. RELEVANCE TO CLINICAL PRACTICE: Protecting the privacy of patients with impaired consciousness in ICUs requires structured care routines, standardised bedside communication, minimisation of unnecessary exposure during procedures and targeted ethical awareness training to support dignity in everyday practice.

این مطالعه کیفی به بررسی تجربیات پرستاران مراقبت‌های ویژه در حفظ حریم خصوصی بیماران با هوشیاری کاهش‌یافته می‌پردازد. یافته‌ها نشان می‌دهد که اولویت‌های بالینی و محدودیت‌های ساختاری مانع حفظ حریم خصوصی می‌شوند. بیماران با هوشیاری کاهش‌یافته به دلیل محدودیت در دفاع از خود، به طور بالقوه در معرض نقض حریم خصوصی قرار دارند. محافظت از این بیماران نیازمند روتین‌های مراقبت استاندارد و آموزش‌های اخلاقی است.

زمینه: حفظ حریم خصوصی در واحدهای مراقبت‌های ویژه (ICU) به دلیل بار کاری بالا، اقدامات تهاجمی، نظارت مداوم و محیط‌های مراقبت مشترک چالش‌برانگیز است. بیماران با هوشیاری کاهش‌یافته ممکن است به دلیل محدودیت در بیان ترجیحات حریم خصوصی، تعیین مرزهای شخصی و دفاع از خود، آسیب‌پذیرتر باشند. هدف: بررسی تجربیات پرستاران مراقبت‌های ویژه در حفظ حریم خصوصی در مراقبت از بیماران با هوشیاری کاهش‌یافته. طراحی مطالعه: یک مطالعه کیفی توصیفی با ۲۰ پرستار مراقبت‌های ویژه در ترکیه بین مارس و آوریل ۲۰۲۶ انجام شد. داده‌ها از طریق مصاحبه‌های نیمه‌ساختاریافته آنلاین جمع‌آوری و با استفاده از تحلیل موضوعی استنتاجی تحلیل شدند. یافته‌ها: چهار موضوع شناسایی شد: حاشیه‌نشینی حریم خصوصی در اولویت‌های بالینی؛ محدودیت‌های ساختاری و فرایندی حریم خصوصی در ICU؛ از دست رفتن موضوعیت و مکانیزه شدن مراقبت در بیماران با هوشیاری کاهش‌یافته؛ و پیامدهای اخلاقی و روان‌اجتماعی نقض حریم خصوصی. نتیجه‌گیری: حریم خصوصی در ICU نه تنها توسط آگاهی اخلاقی فردی، بلکه توسط بار کاری، شرایط محیطی، روتین‌های مراقبت و شیوه‌های ارتباطی شکل می‌گیرد. بیماران با هوشیاری کاهش‌یافته حق مشابهی به حریم خصوصی و کرامت دارند؛ با این حال، توانایی کاهش‌یافته آن‌ها برای بیان ترجیحات، تعیین مرزهای شخصی و دفاع از خود ممکن است نقض حریم خصوصی را کمتر قابل مشاهده کند و وابستگی آن‌ها به پرستاران برای محافظت را افزایش دهد. ارتباط با عملکرد بالینی: محافظت از حریم خصوصی بیماران با هوشیاری کاهش‌یافته در ICU نیازمند روتین‌های مراقبت ساختاریافته، ارتباط استاندارد در نزدیکی تخت، کاهش مواجهه‌های غیرضروری در حین اقدامات و آموزش هدفمند آگاهی اخلاقی برای حمایت از کرامت در عمل روزمره است.
