Esaxerenone attenuates renal injury by inhibiting NR3C2-mediated inflammatory and fibrotic pathways.

چکیده اصلی

Renal inflammation and fibrosis are pathological features of various types of kidney diseases. High-salt diet and aldosterone are risk factors for inducing kidney disease. Esaxerenone is the mineralocorticoid receptor blocker which is effective in treating hypertension. The aim of this study is to observe the effect of esaxerenone on renal inflammation in a rat model of high-salt diet plus aldosterone infusion. We found that rats treated with high-salt diet plus aldosterone showed more serious renal pathological injury. As well as, the expression of kidney injury molecule-1, toll-like receptor 4 (TLR4), phospho-nuclear factor kappa-B‌ (p-NF-κB), F4/80, interleukin-6, ‌tumor necrosis factor-alpha, and interferon gamma were increased (p < 0.05). In addition, high-salt diet plus aldosterone could upregulate the expression of transforming growth factor-beta‌ (TGF-β), p-Smad2, p-Smad3, alpha-smooth muscle actin, vimentin, and collagen I (p < 0.05). However, administration of esaxerenone could ameliorate high-salt diet plus aldosterone - induced these expressions and renal pathological injury (p < 0.05). These data suggest that esaxerenone may alleviate high-salt plus aldosterone-induced renal inflammation and fibrosis by regulating the TLR4/NF-κB and TGF-β/Smad signaling pathways.

این مطالعه نشان می‌دهد که اساکسرنون در مدل موش با رژیم غذایی غنی از نمک و تزریق آلدوسترون، التهاب و فیبروز کلیوی را کاهش می‌دهد. این اثر احتمالاً از طریق مهار مسیرهای سیگنالینگ TLR4/NF-κB و TGF-β/Smad رخ می‌دهد. نتایج نشان می‌دهد که اساکسرنون می‌تواند بیان مولکول‌های التهابی و فیبروتیک را کاهش دهد.

التهاب و فیبروز کلیه ویژگی‌های پاتولوژیک انواع بیماری‌های کلیوی هستند. رژیم غذایی غنی از نمک و آلدوسترون عوامل خطرناک برای ایجاد بیماری کلیوی هستند. اساکسرنون یک مهارکننده گیرنده کورتیکواستروئیدی معدنی است که در درمان فشار خون مؤثر است. هدف این مطالعه مشاهده اثر اساکسرنون بر التهاب کلیه در مدل موش رژیم غذایی غنی از نمک به همراه تزریق آلدوسترون است. ما یافتیم که موش‌های درمان‌شده با رژیم غذایی غنی از نمک و آلدوسترون آسیب پاتولوژیک کلیه‌ای شدیدتری نشان دادند. علاوه بر این، بیان مولکول آسیب کلیه-۱، گیرنده تال‌لایک ۴ (TLR4)، فسفوریل‌نوکلئوز فاکتور کاپا-بی (p-NF-κB)، F4/80، اینترلوکین ۶، فاکتور نکروز تومور آلفا و اینترفرون گاما افزایش یافت (p < 0.05). علاوه بر این، رژیم غذایی غنی از نمک و آلدوسترون می‌توانست بیان عامل تبدیل رشد بتا (TGF-β)، p-Smad2، p-Smad3، آلفا-آکتین میوسی صاف، ویمنتین و کلاژن ۱ را افزایش دهد (p < 0.05). با این حال، تزریق اساکسرنون می‌توانست این بیان‌ها و آسیب پاتولوژیک کلیوی ناشی از رژیم غذایی غنی از نمک و آلدوسترون را بهبود بخشد (p < 0.05). این داده‌ها نشان می‌دهند که اساکسرنون ممکن است با تنظیم مسیرهای سیگنالینگ TLR4/NF-κB و TGF-β/Smad، التهاب و فیبروز کلیوی ناشی از رژیم غذایی غنی از نمک و آلدوسترون را تسکین دهد.
