چکیده اصلی

BACKGROUND: Acute motor and sensory axonal neuropathy is a rare, severe variant of Guillain-Barré syndrome marked by rapid progression, autonomic dysfunction, and delayed recovery. The patient in this report was treated in a resource-limited intensive care unit and had coexisting tropical infections that complicated diagnosis and treatment. CLINICAL FINDINGS: A 40-year-old man presented with acute quadriparesis, respiratory distress, and autonomic instability following a febrile illness. Laboratory tests revealed anemia, electrolyte imbalance, and methicillin-resistant Staphylococcus aureus bacteremia. Serology test results were weakly positive for dengue virus, typhoid, and scrub typhus. Nerve conduction studies confirmed acute motor and sensory axonal neuropathy. DIAGNOSIS: Acute motor and sensory axonal neuropathy was complicated by polyinfection and tuberculous pleuritis, confirmed via cartridge-based nucleic acid amplification testing. Imaging showed bilateral pleural effusion and right lung collapse and consolidation. INTERVENTIONS: Despite 5 cycles of intravenous immunoglobulin therapy, the patient showed no initial improvement and received 7 plasmapheresis cycles. Additional interventions included tracheostomy, vasopressors, bromocriptine for central fever, and antitubercular therapy. Nursing care grounded in the Roy Adaptation Model emphasized airway management, prevention of secondary complications, emotional reassurance, caregiver communication, and coordination of interprofessional care. OUTCOMES: The patient demonstrated steady neurological improvement, was successfully weaned from ventilation, and transitioned to rehabilitation. Nurse-led, theory-informed interventions were key to patient stabilization and long-term recovery. CONCLUSION: This case illustrates the complexity of acute motor and sensory axonal neuropathy with multiple infections and the role of structured nursing care for positive outcomes in intensive care units.

بیماری عصبی آکسونال حاد حرکتی و حسی یک نوع نادر و شدید از سندرم گیلین-باریه است که با پیشرفت سریع، اختلالات خودکار و بهبود تأخیری مشخص می‌شود. بیمار در این گزارش در یک واحد مراقبت‌های ویژه با منابع محدود درمان شد و عفونت‌های استوایی همراهی داشت که تشخیص و درمان را پیچیده کرد. بیمار ۴۰ ساله با فلج چهارطرفه حاد، تنفس دشوار و ناپایداری خودکار پس از یک بیماری تب‌دار ظاهر شد. آزمایش‌های آزمایشگاهی کم‌خونی، ناهماهنگی الکترولیت و باکتری‌هماتومای استافیلوکوک اورئوس مقاوم به متیکیلین را نشان دادند. نتایج سرولوژی برای ویروس دنگی، تب طاعون و تب خاکستری ضعیف مثبت بودند. مطالعات هدایت عصبی بیماری عصبی آکسونال حاد حرکتی و حسی را تأیید کرد. تشخیص بیماری عصبی آکسونال حاد حرکتی و حسی با عفونت‌های متعدد و پلی‌آبسنس پلوریت باکتریالی و با استفاده از تست‌های تشخیصی تقویت‌شونده اکستراکسیون اسید نوکلئیک مبتنی بر کارتریج تایید شد. تصویربرداری نشان‌دهنده تجمع مایع پلورال دوطرفه و سقوط و تجمع ریه راست بود. با وجود ۵ چرخه درمان ایمونوگلوبولین داخل وریدی، بیمار در ابتدا بهبودی نشان نداد و ۷ چرخه پلاسمافریز دریافت کرد. اقدامات دیگر شامل تراشوستومی، داروهای فشارخون، بروموکریپتین برای تب مرکزی و درمان ضد سل بود. مراقبت‌های پرستارانه مبتنی بر مدل تطبیق روی بر مدیریت راه هوایی، پیشگیری از عوارض ثانویه، اطمینان عاطفی، ارتباط با مراقب و هماهنگی مراقبت‌های حرفه‌ای تأکید داشت. بیمار بهبود عصبی پایداری نشان داد، موفق به قطع تنفس مصنوعی شد و به بازتوانی منتقل شد. مداخلات تحت نظارت پرستار و مبتنی بر نظریه کلیدی برای تثبیت بیمار و بهبود طولانی‌مدت بود. این مورد نشان‌دهنده پیچیدگی بیماری عصبی آکسونال حاد حرکتی و حسی با عفونت‌های متعدد و نقش مراقبت‌های ساختاریافته پرستار برای نتایج مثبت در واحدهای مراقبت‌های ویژه است.
