چکیده اصلی

Surgical site infection is one of the most frequent post-operative consequences are infections caused by invasive surgical procedures that occur within 30 days if the patient does not have a prosthetic in place or within a year if a prosthetic is implanted in the patient. Many Gram-positive and Gram-negative bacteria are causing surgical site infections. The objective of this study was to isolate and identify the Gram-Positive Cocci and their antibiotic susceptibility patterns from patients with delayed surgical site infections in Mymensingh Medical College Hospital of Bangladesh. This cross-sectional study involved 155 patients suffering from postsurgical wound infection more than three weeks as respondents. The study was conducted in the Department of Microbiology, Mymensingh Medical College, Bangladesh from March 2024 to February 2025. The bacterial pathogens were isolated from pus/ wound swab samples obtained from those patients. The Gram-Positive Cocci were identified through several standard biochemical tests and finally, the culture sensitivity tests of those bacterial isolates were performed by disk diffusion method. Data was documented using standard questionnaires. The mean age of the patients in the study was 33.97 years. Most of the patients 96(61.9%) were female. Out of a total 155 samples, 143(92.30%) were positive by culture. Among them Gram positive were 16(11.2%); Staphylococcus aureus was 11(68.75%), coagulase negative Staphylococcus (CONS) was 4(25.0%) and Enterococcus spp. was 1(6.25%). Among the isolated Staphylococcus aureus, 6(54.54%) were found MRSA. The Staphylococcus aureus showed 100.0% sensitivity to meropenem and linezolid and 100.0% resistant to Ceftriaxone. The Coagulase negative Staphylococcus isolates were 100.0% sensitive to cefoxitin and showed resistant to most the drugs. The Enterococcus spp. was sensitive to amoxiclav, linezolid and Pipercillin-Tazobactum. Meropenem and linezolid have shown better activity on to S. aureus. CoNS exhibited higher resistance to many antibiotics compared to S. aureus.

این مطالعه مقطعی ۱۵۵ بیمار با عفونت زخم پس از جراحی دیررس را بررسی کرد. ۹۲.۳٪ نمونه‌ها مثبت بودند و ۱۱.۲٪ آن‌ها باکتری‌های گرم مثبت بودند. استافیلوکوکوس آورئوس شایع‌ترین عامل بود و ۵۴.۵۴٪ آن‌ها MRSA بودند. استافیلوکوکوس منفی کوآژولاز و آنتروکوک اسپس نیز شناسایی شدند. استافیلوکوکوس آورئوس ۱۰۰٪ حساس به مروپنم و لینزولید بود و ۱۰۰٪ مقاوم به سترپتومایسین. استافیلوکوکوس منفی کوآژولاز ۱۰۰٪ حساس به سافوکسیمین بود.

عفونت محل جراحی یکی از شایع‌ترین پیامدهای پس از جراحی است که در ۳۰ روز اول پس از عمل رخ می‌دهد. هدف این مطالعه جداسازی و شناسایی باکتری‌های گرم مثبت و الگوهای حساسیت آن‌ها به آنتی‌بیوتیک در بیماران مبتلا به عفونت محل جراحی دیررس در بیمارستان کالج پزشکی میمنسینگ بنگلادش بود. این مطالعه مقطعی شامل ۱۵۵ بیمار مبتلا به عفونت زخم پس از جراحی بیش از سه هفته بود. مطالعه در بخش میکروبیولوژی کالج پزشکی میمنسینگ از مارس ۲۰۲۴ تا فوریه ۲۰۲۵ انجام شد. باکتری‌ها از نمونه‌های پانسمان زخم یا نمونه‌های کشت استریل جداسازی شدند. باکتری‌های گرم مثبت با تست‌های بیوشیمیایی استاندارد شناسایی شدند و حساسیت آن‌ها با روش پخش دیسک بررسی شد. میانگین سنی بیماران ۳۳.۹۷ سال بود و ۹۶ نفر (۶۱.۹٪) زن بودند. از ۱۵۵ نمونه، ۱۴۳ (۹۲.۳٪) مثبت بودند. باکتری‌های گرم مثبت ۱۶ مورد (۱۱.۲٪) بودند: استافیلوکوکوس آورئوس ۱۱ مورد (۶۸.۷۵٪)، استافیلوکوکوس منفی کوآژولاز ۴ مورد (۲۵.۰٪) و آنتروکوک اسپس ۱ مورد (۶.۲۵٪). از ۱۱ مورد استافیلوکوکوس آورئوس، ۶ مورد (۵۴.۵۴٪) MRSA بودند. استافیلوکوکوس آورئوس ۱۰۰٪ حساس به مروپنم و لینزولید و ۱۰۰٪ مقاوم به سترپتومایسین بود. استافیلوکوکوس منفی کوآژولاز ۱۰۰٪ حساس به سافوکسیمین و مقاوم به اکثر داروها بود. آنتروکوک اسپس حساس به آموکسی‌کلاو، لینزولید و پیرکیلین-تازوباکتام بود. مروپنم و لینزولید فعالیت بهتری در برابر S. aureus داشتند. CoNS مقاومت بالاتری نسبت به S. aureus به بسیاری از آنتی‌بیوتیک‌ها نشان داد.

روش پژوهش

این مطالعه مقطعی در بخش میکروبیولوژی کالج پزشکی میمنسینگ انجام شد. باکتری‌ها از نمونه‌های زخم با روش کشت و تست‌های بیوشیمیایی استاندارد جداسازی و شناسایی شدند. حساسیت آنتی‌بیوتیکی با روش پخش دیسک بررسی شد.

محدودیت‌ها

محدودیت‌های مطالعه شامل عدم گزارش DOI و عدم دسترسی به متن کامل مجله است. همچنین، نتایج فقط در یک بیمارستان محلی (میمنسینگ) به دست آمده است.
