چکیده اصلی

Pathologic tooth migration (PTM), tooth flaring, and severe mobility are frequent clinical problems in stage IV periodontitis and can cause major aesthetic and functional impairment. Conventional correction often requires prolonged interdisciplinary periodontal-orthodontic treatment. This case series introduces the PIANO protocol (Periodontal Immediate Adhesive Nonsurgical Orthorepositioning), a splint-assisted approach combining orthorepositioning with immediate stabilization during step 2 periodontal therapy. Three patients with generalized stage IV, grade C periodontitis, anterior PTM, and grade III mobility were treated. Before local anaesthesia, the affected teeth were moved with gentle digital pressure towards the closest passively achievable functional and aesthetic position. Movement was discontinued if resistance or discomfort occurred. The teeth were then adhesively splinted, premature occlusal contacts were adjusted, and nonsurgical periodontal therapy was performed. At 6 months, all treated teeth remained clinically stable and responded positively to pulp sensibility testing. Periodontal inflammation had resolved, no residual pathological probing depths were detected, and radiographs showed defect fill compatible with osseous repair. Patient-reported outcomes were favourable, particularly for aesthetics, function, and willingness to undergo the procedure again. No pulpal complications or splint failures occurred. Within the limitations of this three-patient case series, the PIANO protocol provided rapid aesthetic and functional improvement while facilitating periodontal treatment of grade III mobile teeth. It may represent a minimally invasive adjunct for selected patients seeking rapid aesthetic improvement and a time- and cost-efficient treatment option.

جابجایی نامناسب دندان (PTM)، پهن شدن دندان و لقای شدید دندان مشکلات بالینی شایعی در پریودنتیت مرحله IV هستند که می‌توانند باعث اختلالات زیبایی و عملکردی جدی شوند. اصلاح معمولی اغلب نیاز به درمان طولانی‌مدت بین‌رشته‌ای پریودنتیک-ارتودنسی دارد. این سری مورد معرفی پروتکل پیانو (اصلاح فوری و غیرجراحی ارتودنسی با چسب)، یک رویکرد کمکی با گیره است که اصلاح موقعیت را با تثبیت فوری در مرحله دوم درمان پریودنتیک ترکیب می‌کند. سه بیمار با پریودنتیت مرحله IV عمومی، درجه C، PTM قدامی و لقای درجه III درمان شدند. قبل از بیهوشی محلی، دندان‌های درگیر با فشار ملایم دیجیتال به سمت نزدیک‌ترین موقعیت عملکردی و زیبایی قابل دستیابی غیرفعال حرکت داده شدند. حرکت متوقف شد اگر مقاومت یا ناراحتی رخ داد. سپس دندان‌ها با گیره چسبی بسته شدند، تماس‌های دندانی زودرس تنظیم شد و درمان پریودنتیک غیرجراحی انجام شد. در ۶ ماه، تمام دندان‌های درمان‌شده پایداری بالینی داشتند و به تست حساسیت پالپ پاسخ مثبت دادند. التهاب پریودنتیک حل شده بود، هیچ عمق سنجش پاتولوژیک باقی‌مانده‌ای شناسایی نشد و عکس‌برداری رادیوگرافیک نشان‌دهنده پر شدن نقص سازه‌ای سازگار با ترمیم استخوان بود. نتایج گزارش‌شده توسط بیمار مطلوب بود، به ویژه از نظر زیبایی، عملکرد و میزان تمایل به تکرار پروسه. هیچ عارضه پالپ یا شکست گیره‌ای رخ نداد. در محدودیت‌های این سری مورد سه نفره، پروتکل پیانو بهبود سریع زیبایی و عملکرد را فراهم کرد و درمان پریودنتیک دندان‌های لقای درجه III را تسهیل نمود. این می‌تواند یک کمکی کم‌تهاجمی برای بیماران منتخب باشد که به بهبود سریع زیبایی و گزینه‌ای زمان‌بر و هزینه‌بر نیاز دارند.
