چکیده اصلی

BACKGROUND AND AIMS: Alcohol use disorder (AUD) and alcohol associated liver disease (ALD) commonly co-occur, and integrated approaches to treatment are associated with better outcomes. This study aimed to develop an online module, the Program for Advanced Training of Hepatologists in Alcohol Use Disorder Management (PRATHAM), to enroll and train hepatologists, and to evaluate changes in participants' knowledge, attitudes, practices, and self-perceived competence pre- and post-training. METHODS: This included identification of specific needs, establishing learning objectives, designing a curriculum, and developing didactic presentations for hepatologists. Interested participants were enrolled in the online module and were asked to submit pre-tests, clinical cases, and a post-test, which included an evaluation of knowledge, attitudes, practice, and self-perceived competence. Descriptive statistics and the Wilcoxon signed-rank test were used to analyze the data. An intention-to-treat analysis was used to address missing data. RESULTS: An 8-week curriculum was developed for 18 hepatologists from five different countries. Of these, 17 completed the pre-test, and 11 filled out the post-test proforma. Eleven complex clinical cases were discussed. There was a significant improvement in the participants' knowledge, practice, and self-perceived competence pre- and post-training. However, no significant change in attitudes was noted. CONCLUSIONS: This study underscores the importance of tailored educational interventions in medical specialties such as hepatology, where practitioners frequently encounter patients with AUD. The significant improvements in practice, self-efficacy, and knowledge highlight the potential of targeted training programs to enhance patient care. Future studies could engage a more diverse group of hepatologists and evaluate patient outcomes.

Acute kidney injury at a tertiary nephrology referral center: predictors of renal recovery and in-hospital mortality.

BACKGROUND: Acute kidney injury (AKI) is a global health problem associated with substantial morbidity, mortality, and healthcare costs. Despite advances in prevention and management, in-hospital mortality remains high, particularly among critically ill patients. OBJECTIVES: To determine incidence, risk factors of AKI among patients admitted to Mansoura Nephrology and Dialysis Unit (MNDU), a tertiary nephrology referral center in Dakah…

Prevalence, risk factors and renoprotective treatment of chronic kidney disease among internal medicine outpatients at a Central European tertiary care centre: a cross-sectional study.

OBJECTIVES: To determine the prevalence of chronic kidney disease (CKD) in internal medicine outpatients in a Central European tertiary care setting, identify associated risk factors and assess use and potential impact of renoprotective therapies. We hypothesised that CKD is common, underdiagnosed and undertreated. DESIGN: Cross-sectional observational study. SETTING: Tertiary care outpatient clinics in Central Europe. PARTICIPANTS: 19…

Development and Nationwide Multicentre Evaluation of Guideline-Grounded Large Language Model Chatbots to Support Patient Self-Management and Education in Rheumatology.

Patients with rheumatic diseases have persistent information needs that are not fully addressed in routine care. We developed and evaluated guideline-grounded, large language model (LLM) chatbots to support patient self-management and education in rheumatology.Ten disease-specific chatbots based on German guidelines were co-developed and deployed through 13 rheumatology centres and six patient organisations. Chatbot users rated respons…

Discordance between left ventricular ejection fraction and global longitudinal strain during routine cardio-oncology surveillance.

BACKGROUND: Left ventricular ejection fraction (EF) and global longitudinal strain (GLS) are widely used for cardiac surveillance in patients receiving potentially cardiotoxic cancer therapy. While GLS is more sensitive for detecting early myocardial dysfunction, the relationship between EF and GLS in routine clinical practice remains incompletely characterized, and discordance between these parameters may complicate clinical interpret…
