چکیده اصلی

BACKGROUND: Uncontrolled hypertension remains a leading risk factor for cardiovascular disease, particularly in rural and medically underserved populations. Remote patient monitoring (RPM) and protocol-driven care models have emerged as promising strategies to improve blood pressure (BP) control. This study evaluated a pharmacist-led RPM program for patients with uncontrolled hypertension integrated into routine clinical practice. METHODS: A retrospective chart review was conducted among adults with uncontrolled hypertension enrolled in an RPM program at a large academic medical center between 2018 and 2022. The program incorporated Bluetooth-enabled BP monitors, daily registered nurse (RN) monitoring, and pharmacist-led medication management guided by a standardized treatment protocol. Propensity score matching was used to compare clinic BP outcomes with a control group receiving usual in-clinic care. RESULTS: Of 471 patients who actively participated, 209 (44.4%) completed the program and were included in the primary analysis. Propensity score matching identified 201 matched pairs with balanced baseline characteristics. Compared with controls, RPM participants achieved greater reductions in systolic BP (-14.9 vs. -8.2 mmHg; p < 0.001) and diastolic BP (-5.9 vs. -2.2 mmHg; p = 0.002). CONCLUSION: A pharmacist-led, protocol-driven RPM program was associated with greater reductions in BP compared with those receiving in-clinic care alone. These findings support the integration of pharmacists and structured treatment algorithms in telehealth-enabled hypertension management. Improving patient engagement and addressing reimbursement challenges remain essential for long-term program sustainability.
