چکیده اصلی

CONTEXT: Postoperative pain and persistent microbial contamination remain significant challenges in pediatric endodontics following pulpectomy. Intracanal cryotherapy has emerged as a promising adjunct because of its anti-inflammatory, analgesic, and antimicrobial properties. However, its effectiveness requires further clinical evaluation. AIM: The aim of the study was to compare the effect of intracanal cryotherapy using cold saline (2°C-4°C) with room temperature saline (25°C-30°C) on postoperative pain and antimicrobial efficacy during the pulpectomy procedure in primary teeth. SETTINGS AND DESIGN: A randomized clinical trial was conducted involving 30 children (3-7 years) requiring pulpectomy in primary teeth. MATERIALS AND METHODS: Participants were randomly allocated into two groups: Group I (room temperature saline) and Group II (cryotherapy with cold saline). Postoperative pain was assessed using the Wong-Baker Faces Pain Rating Scale at 6, 24, 48, and 72 h posttreatment. Antimicrobial efficacy was evaluated by comparing colony-forming units (CFUs) before and after irrigation via microbiological analysis. STATISTICAL ANALYSIS: Data were analyzed using SPSS v26.0. Paired t - tests/Wilcoxon for intragroup; independent t -test/Mann-Whitney for intergroup were applied. RESULTS: Both groups showed significant intragroup pain reduction over 6, 24, 48, and 72 h ( P < 0.05), with cryotherapy exhibiting lower mean scores but no intergroup differences ( P > 0.05). Microbial counts decreased significantly within groups ( P = 0.001: Group I: 4.23-2.21 and Group II: 3.69-0.68), with cryotherapy superior postintervention ( P < 0.001). No baseline differences were existed. CONCLUSIONS: Intracanal cryotherapy with cold saline is an effective, simple, and cost-efficient method for reducing postoperative pain and enhancing antimicrobial efficacy in primary teeth pulpectomy.

این مطالعه بالینی تصادفی تأثیر سرم‌درمانی داخل‌کانالی با سرم سرد را در مقایسه با سرم دمای اتاق بر درد پس از عمل و اثربخشی آنتی‌میکروبی در دندان‌های شیری بررسی کرد. نتایج نشان داد کاهش معنی‌دار درد و تعداد میکروارگانیسم‌ها در هر دو گروه، با اینکه سرم سرد امتیازات درد کمتری داشت اما تفاوت معنی‌داری بین گروه‌ها وجود نداشت.

پس از پالپکتومی در دندان‌های شیری، درد پس از عمل و آلودگی باکتریایی باقی‌مانده چالش‌های مهمی هستند. سرم‌درمانی داخل‌کانالی به دلیل خواص ضدالتهابی، ضد درد و آنتی‌میکروبی، روشی امیدوارکننده است. هدف این مطالعه مقایسه تأثیر سرم‌درمانی با سرم سرد (۲-۴ درجه سانتی‌گراد) در برابر سرم دمای اتاق (۲۵-۳۰ درجه سانتی‌گراد) بر درد پس از عمل و اثربخشی آنتی‌میکروبی در حین پالپکتومی دندان‌های شیری بود. این مطالعه بالینی تصادفی شامل ۳۰ کودک (۳-۷ سال) بود که پالپکتومی در دندان‌های شیری نیاز داشتند. درد پس از عمل با مقیاس درد صورت‌های وونگ-بکر در ۶، ۲۴، ۴۸ و ۷۲ ساعت ارزیابی شد. اثربخشی آنتی‌میکروبی با مقایسه واحدهای تشکیل‌دهنده کلونی (CFU) قبل و بعد از شستشو ارزیابی شد. نتایج نشان داد کاهش معنی‌دار درد در هر دو گروه در طول زمان و کاهش معنی‌دار تعداد میکروارگانیسم‌ها، با اینکه سرم سرد امتیازات میانگین کمتری داشت اما تفاوت معنی‌داری بین گروه‌ها وجود نداشت. نتیجه‌گیری: سرم‌درمانی داخل‌کانالی با سرم سرد روشی مؤثر، ساده و اقتصادی برای کاهش درد پس از عمل و بهبود اثربخشی آنتی‌میکروبی در پالپکتومی دندان‌های شیری است.

روش پژوهش

این مطالعه یک آزمایش بالینی تصادفی با ۳۰ کودک (۳-۷ سال) بود که پالپکتومی دندان‌های شیری نیاز داشتند. شرکت‌کنندگان به دو گروه تصادفی: گروه اول (سرم دمای اتاق) و گروه دوم (سرم‌درمانی با سرم سرد) تقسیم شدند. داده‌ها با استفاده از SPSS v26.0 تحلیل شدند.

محدودیت‌ها

مطالعه دارای حجم نمونه کوچک (۳۰ نفر) بود. تفاوت‌های بین‌گروهی در درد پس از عمل معنی‌دار نبود. نتایج ممکن است در دندان‌های دائمی یا شرایط دیگر اعمال نشود.
