چکیده اصلی

The role of cerebellar glutamatergic dysfunction in cognitive impairment of Parkinson's disease (PD) remains poorly understood. Glutamate chemical exchange saturation transfer (GluCEST) imaging enables noninvasive assessment of regional glutamate levels. To investigate the associations between cerebellar GluCEST-derived alterations and cognitive impairment as well as other clinical characteristics in patients with PD, 64 PD patients underwent 3 T MRI including GluCEST sequence and clinical assessments. Patients were divided into PD with preserved cognition (PD-CN, MoCA > 22, n = 23) and PD with impaired cognition (PD-CI, MoCA ≤ 22, n = 41) subgroups. GluCEST-derived parameters including MTRasym, CESTRnr, and MTRRex were obtained from the bilateral cerebellar cortex and white matter. Between-group differences in the GluCEST-derived parameters were assessed using ANOVA, adjusting for age and sex. Pearson or Spearman correlation analyses were used to examine associations between GluCEST-derived parameters and cognitive performance. Benjamini-Hochberg false discovery rate (FDR) correction was applied for multiple comparisons. Compared with the PD-CN group, the PD-CI group showed lower MTRasym values in the bilateral cerebellar cortex (CC) and the right cerebellar white matter (CWM) (p < 0.05) and a lower CESTRnr value in the right CWM (p = 0.016). Within the PD-CI group, MoCA total score was negatively correlated with MTRRex in the left CC (p = 0.013). Attention/working memory was negatively correlated with CESTRnr (p = 0.021) and MTRRex (p = 0.007) in the left CC. Orientation was negatively correlated with MTRRex in the left CWM, right CC, and right CWM (p < 0.05). In addition, negative correlation between disease duration and MTRRex in the right CWM remained significant after FDR correction (p = 0.020). Cerebellar GluCEST alterations are associated with cognitive impairment in PD. GluCEST may serve as a possible neuroimaging marker in PD.

این مطالعه نشان داد که تغییرات GluCEST در کوره‌ی مغز با اختلال شناختی در بیماری پارکینسون مرتبط است. بیماران با اختلال شناختی مقادیر پایین‌تری از پارامترهای GluCEST را نشان دادند. نمره MoCA و عملکردهای شناختی خاص با پارامترهای GluCEST در مناطق مختلف کوره‌ی مغز رابطه منفی داشتند.

نقش اختلال گلوتامرژیک کوره‌ای در اختلال شناختی بیماری پارکینسون (PD) هنوز به درستی شناخته نشده است. تصویربرداری GluCEST امکان ارزیابی غیرتهاجمی سطوح منطقه‌ای گلوتامات را فراهم می‌کند. این مطالعه به بررسی ارتباط تغییرات حاصل از GluCEST کوره‌ای با اختلال شناختی و ویژگی‌های بالینی در ۶۴ بیمار مبتلا به PD پرداخت. بیماران به دو زیرگروه PD با حفظ شناخت (PD-CN) و PD با اختلال شناختی (PD-CI) تقسیم شدند. تغییرات بین گروهی در پارامترهای GluCEST با استفاده از ANOVA بررسی شد. نتایج نشان داد که گروه PD-CI مقادیر پایین‌تری از MTRasym در قشر کوره‌ای دوطرف و سفید مغز کوره‌ی راست و مقادیر پایین‌تری از CESTRnr در سفید مغز کوره‌ی راست داشت. در گروه PD-CI، نمره کل MoCA با MTRRex در قشر کوره‌ی چپ رابطه منفی داشت. توجه/حافظه کاری با CESTRnr و MTRRex در قشر کوره‌ی چپ رابطه منفی داشت. جهت‌یابی با MTRRex در سفید مغز کوره‌ی چپ، قشر کوره‌ی راست و سفید مغز کوره‌ی راست رابطه منفی داشت. مدت بیماری با MTRRex در سفید مغز کوره‌ی راست رابطه منفی داشت.

روش پژوهش

این مطالعه یک مطالعه توصیفی-آزمایشی با استفاده از تصویربرداری 3 تی MRI و GluCEST در ۶۴ بیمار مبتلا به PD انجام شد. بیماران بر اساس نمره MoCA به دو گروه تقسیم شدند.

محدودیت‌ها

محدودیت‌های این مطالعه شامل عدم وجود گروه کنترل سالم و عدم بررسی سایر عوامل شناختی است.
