چکیده اصلی

Autistic women experience increased obstetric and perinatal risks, partly driven by communication barriers within maternity care. Evidence on how these barriers are experienced in low- and middle-income countries remains scarce. This study aimed to analyze communication barriers in perinatal care based on narratives of autistic women who experienced pregnancy, childbirth, and postpartum care in Brazil. A qualitative study was conducted using the Hermeneutic-Dialectic Circle (HDC) approach. Thirty-four autistic women (22 with support level 1 and 12 with support level 2, including three non-speaking participants with complex communication needs) were recruited through convenience sampling, supplemented by active recruitment through participant referral, and participated in semi-structured interviews conducted in multiple accessible formats (video call, real-time messaging, in-person, and augmentative and alternative communication). Data were analyzed using constant comparative methods within the hermeneutic-dialectic framework, guided by the social model of disability and the neurodiversity paradigm. Three central categories were constructed: (1) communicational demands of autistic women and their non-recognition, including alexithymia, sensory hypersensitivity, and need for predictability; (2) institutional and interpersonal communication failures, encompassing symptom invalidation, disrespectful care, and exclusion of non-speaking women; and (3) experiences of respectful and effective communication, demonstrating that adapted strategies promote safety and autonomy. Participants' narratives indicate that communication barriers in perinatal care for autistic women operate systemically rather than individually. Training healthcare professionals and implementing accessible communication strategies are essential to promote respectful, safe, and inclusive maternity care.

این مطالعه کیفی بر تجربه زنان اوتیستیک از موانع ارتباطی در مراقبت‌های دوران بارداری در برزیل تمرکز دارد. یافته‌ها نشان می‌دهد که این موانع به صورت سیستماتیک عمل می‌کنند و شامل عدم شناخت نیازهای ارتباطی، مراقبت بی‌احترامانه و حذف زنان غیرکلامی می‌شوند. استراتژی‌های ارتباطی سازگار برای تضمین ایمنی و استقلال ضروری هستند.

زنان اوتیستیک ریسک‌های زایشی و دوران پس از زایمان بیشتری را تجربه می‌کنند که بخشی از آن‌ها ناشی از موانع ارتباطی در مراقبت‌های مامایی است. شواهد کمی در مورد نحوه تجربه این موانع در کشورهای با درآمد پایین و متوسط وجود دارد. این مطالعه به تحلیل موانع ارتباطی در مراقبت‌های دوران بارداری بر اساس روایت زنان اوتیستیک که بارداری، زایمان و مراقبت‌های پس از زایمان را در برزیل تجربه کرده‌اند، پرداخت. این مطالعه کیفی با رویکرد دایره هیرمنوتیک-دیالکتیک (HDC) انجام شد. سی و چهار زن اوتیستیک (۲۲ نفر با سطح حمایت ۱ و ۱۲ نفر با سطح حمایت ۲، شامل سه شرکت‌کننده غیرکلامی با نیازهای ارتباطی پیچیده) از طریق نمونه‌گیری آسان و با استخدام فعال از طریق معرفی شرکت‌کننده، جذب شدند و در مصاحبه‌های نیمه‌ساخت‌یافته که در قالب‌های دسترسی‌پذیر مختلف (تماس ویدیویی، پیام‌رسانی لحظه‌ای، حضوری و ارتباط جایگزین و جایگزین) برگزار شد، شرکت کردند. داده‌ها با استفاده از روش‌های مقایسه مداوم در چارچوب هیرمنوتیک-دیالکتیک، با هدایت مدل اجتماعی معلولیت و پارادایم تنوع عصبی، تحلیل شدند. سه دسته اصلی ساخته شد: (۱) تقاضاهای ارتباطی زنان اوتیستیک و عدم شناخت آن‌ها، شامل آلکسیتیمی، حساسیت بیش از حد حسی و نیاز به پیش‌بینی‌پذیری؛ (۲) شکست‌های ارتباطی نهادی و بین‌فردی، شامل بی‌اعتبارسازی علائم، مراقبت بی‌احترام و حذف زنان غیرکلامی؛ و (۳) تجربیات ارتباط محترمانه و مؤثر، که نشان می‌دهد استراتژی‌های سازگار امنیت و استقلال را ترویج می‌کنند. روایت شرکت‌کنندگان نشان می‌دهد که موانع ارتباطی در مراقبت‌های دوران بارداری برای زنان اوتیستیک به صورت سیستماتیک عمل می‌کنند، نه فردی. آموزش متخصصان مراقبت‌های بهداشتی و پیاده‌سازی استراتژی‌های ارتباطی دسترسی‌پذیر برای ترویج مراقبت مامایی محترمانه، ایمن و شامل، ضروری است.
