چکیده اصلی

This scoping review examines and synthesizes the theoretical foundations underpinning trauma-informed care (TIC) in mental health settings. A systematic search of CINAHL, Scopus, Web of Science, and PubMed yielded 1278 records, of which 23 peer-reviewed studies published between 2010 and 2025 met the inclusion criteria. Reporting followed the PRISMA-ScR (Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews) checklist and explanation. Seven overarching theoretical domains were identified: neurobiological and psychophysiological foundations, psychological and relational models, adverse childhood experiences and social determinants frameworks, SAMHSA and Harris & Fallot principles, socio-ecological and intersectional frameworks, organizational and implementation science models, and integrated or hybrid approaches. These frameworks highlight the multidimensional nature of trauma and its implications for mental health nursing practice, interdisciplinary care, organizational change, and policy development. The findings underscore that TIC is not a single model but a pluralistic and evolving concept bridging individual, relational, and structural dimensions of trauma. Theoretical clarity can enhance implementation fidelity, reduce re-traumatization, and support recovery-oriented care. Future research should focus on evaluating hybrid frameworks and linking theoretical models to measurable individual, organizational, and system-level outcomes.

این مرور نظام‌مند ۲۳ مطالعه را از سال ۲۰۱۰ تا ۲۰۲۵ برای شناسایی مبانی نظری مراقبت مبتنی بر آسیب (TIC) در محیط‌های سلامت روان بررسی می‌کند. هفت حوزه نظری اصلی شامل مبانی عصبی‌روان‌شناختی، مدل‌های روان‌شناختی، عوامل اجتماعی، اصول سازمانی و رویکردهای ترکیبی است. یافته‌ها نشان می‌دهند که TIC یک مفهوم چندگانه است که ابعاد مختلف آسیب را پوشش می‌دهد.

این مرور نظام‌مند، مبانی نظری پشت مراقبت مبتنی بر آسیب (TIC) در محیط‌های سلامت روان را بررسی و ترکیب می‌کند. جستجوی سیستماتیک در CINAHL، Scopus، Web of Science و PubMed منجر به یافتن ۱۲۷۸ رکورد شد، از جمله ۲۳ مطالعه منتشر شده که بین سال‌های ۲۰۱۰ تا ۲۰۲۵ معیارهای ورود را برآورده کردند. هفت حوزه نظری اصلی شناسایی شد: مبانی عصبی‌روان‌شناختی و روان‌فیزیولوژیک، مدل‌های روان‌شناختی و روابطی، چارچوب‌های تجربیات کودکی بد و عوامل اجتماعی تعیین‌کننده، اصول SAMHSA و Harris & Fallot، چارچوب‌های اجتماعی-اکولوژیکی و برخوردی، مدل‌های سازمانی و علم اجرا، و رویکردهای یکپارچه یا ترکیبی. این چارچوب‌ها ماهیت چندبعدی آسیب و پیامدهای آن را برای عمل مراقبت سلامت روان، مراقبت بین‌رشته‌ای، تغییر سازمانی و توسعه سیاست برجسته می‌کنند. یافته‌ها تاکید می‌کنند که TIC یک مدل واحد نیست، بلکه مفهومی چندگانه و در حال تکامل است که ابعاد فردی، روابطی و ساختاری آسیب را پیوند می‌دهد. شفافیت نظری می‌تواند وفاداری اجرا را افزایش دهد، بازآسیب‌زدایی را کاهش دهد و مراقبت متمرکز بر بهبود را پشتیبانی کند. پژوهش‌های آینده باید بر ارزیابی چارچوب‌های ترکیبی و پیوند دادن مدل‌های نظری به نتایج قابل اندازه‌گیری در سطح فردی، سازمانی و سیستماتیک تمرکز کند.
