How Nurses Maintain High-Risk Medication Safety in Paediatric Intensive Care.

چکیده اصلی

BACKGROUND: Existing research has focused mainly on high-risk medication errors, contributory factors and process-based interventions; less is known about nurses' everyday safety work in paediatric intensive care. AIM: To explore how nurses maintain high-risk medication safety in paediatric intensive care. STUDY DESIGN: A descriptive qualitative study using purposive sampling, critical incident interviews and inductive content analysis at a tertiary children's hospital in China. RESULTS: Ten nurses participated. Four themes were generated: anomaly recognition and proactive verification; team communication and continued follow-up after risk identification; safety checks and bedside adaptations in medication practice; and learning from near misses and adjusting future practice. Together, these practices spanned order review, preparation, administration, pump and compatibility management, monitoring and team learning. CONCLUSIONS: Experienced nurses in paediatric intensive care maintained high-risk medication safety through linked practices of anomaly recognition, proactive verification, team communication, continued follow-up, safety checks, bedside adaptation and near-miss learning. RELEVANCE TO CLINICAL PRACTICE: Training and workflow support should strengthen anomaly recognition, dose and infusion-rate recalculation, communication, active double checking, compatibility checking, infusion-pump management, follow-up monitoring and near-miss learning.

این مطالعه کیفی توصیفی، نحوه عملکرد پرستاران در بیمارستان کودکان سطح tertiary چین را برای حفظ ایمنی داروهای پرخطر بررسی می‌کند. نتایج نشان می‌دهد که پرستاران از مجموعه‌ای از عملکردهای پیوسته مانند تشخیص ناهنجاری، تأیید پیشگیرانه، ارتباط تیمی و یادگیری از نزدیک به خطا برای جلوگیری از خطا استفاده می‌کنند. این عملکردها شامل مراحل مختلف از سفارش‌دهی تا تزریق و نظارت می‌شوند.

پیشینه: پژوهش‌های موجود عمدتاً بر خطاهای دارویی پرخطر، عوامل موثر و مداخلات مبتنی بر فرآیند تمرکز دارند؛ اما اطلاعات کمی درباره کار ایمنی روزمره پرستاران در مراقبت‌های اورژانس کودکان وجود دارد. هدف: بررسی نحوه حفظ ایمنی داروهای پرخطر در مراقبت‌های اورژانس کودکان. طراحی مطالعه: یک مطالعه توصیفی کیفی با نمونه‌گیری هدفمند، مصاحبه رویدادهای حیاتی و تحلیل محتوای استنتاجی در یک بیمارستان کودکان سطح tertiary در چین. نتایج: ده پرستار مشارکت کردند. چهار موضوع اصلی شناسایی شد: تشخیص ناهنجاری و تأیید پیشگیرانه؛ ارتباط تیمی و پیگیری مداوم پس از شناسایی ریسک؛ بررسی‌های ایمنی و سازگاری‌های کنار تخت در عمل دارو؛ و یادگیری از نزدیک به خطا و تنظیم عمل آینده. این عملکردها شامل بازبینی سفارش، آماده‌سازی، تزریق، مدیریت پمپ و سازگاری، نظارت و یادگیری تیمی می‌شوند. نتیجه‌گیری: پرستاران باتجربه در مراقبت‌های اورژانس کودکان با عملکردهای پیوسته تشخیص ناهنجاری، تأیید پیشگیرانه، ارتباط تیمی، پیگیری، بررسی‌های ایمنی، سازگاری کنار تخت و یادگیری از نزدیک به خطا، ایمنی داروهای پرخطر را حفظ کردند. اهمیت بالینی: آموزش و حمایت از جریان کار باید تشخیص ناهنجاری، محاسبه مجدد دوز و نرخ تزریق، ارتباط، تأیید دوگانه فعال، بررسی سازگاری، مدیریت پمپ تزریقی، نظارت پیگیری و یادگیری از نزدیک به خطا را تقویت کند.
