چکیده اصلی

BACKGROUND: Midwifery continuity models of care have been associated with positive health outcomes and positive childbirth experiences. Such models may be beneficial for women with fear of birth but remain uncommon in Sweden. AIM: To explore thoughts and perceptions of midwifery continuity of care among women with fear of birth and describe their experiences of fragmented maternity care. METHOD: A qualitative research design in which semi-structured telephone interviews were conducted during pregnancy and postpartum with women scoring above 70 on the Fear of Birth Scale. Data was analyzed using reflexive thematic analysis. RESULTS: Two themes were identified: A desire for knowing and being known and Unheld in the hands of many. These illustrate women's desire to feel secure and their belief that a trusting relationship with a known midwife throughout pregnancy, childbirth and the postpartum period could reduce their fear. Fragmented standard care was perceived as stressful, and all women expressed interest in midwifery continuity of care. CONCLUSION: Women with fear of birth believed that relational continuity with a known midwife could foster trust, guidance and reduce fear. The fragmented standard care was perceived as uncertain, contributing to emotional distress. These findings highlight the need for personalized and relational midwifery care.

این پژوهش کیفی به بررسی تجربه زنانی با ترس از زایمان در مورد مراقبت‌های مامایی پیوسته و پراکنده می‌پردازد. یافته‌ها نشان می‌دهد که زنان میل دارند با یک مامای شناخته‌شده رابطه اعتماد داشته باشند تا ترس خود را کاهش دهند. مراقبت‌های پراکنده استرس‌زا و نامطمئن تلقی می‌شوند. این مطالعه نیاز به مراقبت مامایی شخصی‌سازی‌شده را تأیید می‌کند.

مدل‌های مراقبت مامایی پیوسته با نتایج سلامت مثبت و تجربه‌های زایمان مثبت همراه بوده‌اند. این مدل‌ها ممکن است برای زنانی با ترس از زایمان مفید باشند، اما در سوئد رایج نیستند. هدف این پژوهش بررسی افکار و باورهای زنان دارای ترس از زایمان در مورد مراقبت‌های مامایی پیوسته و توصیف تجربه‌های آن‌ها از مراقبت‌های بارداری پراکنده است. این پژوهش با طراحی کیفی انجام شد و مصاحبه‌های نیمه‌ساختاریافته تلفنی در دوران بارداری و پس از زایمان با زنانی که نمره‌شان در مقیاس ترس از زایمان بالای ۷۰ بود، برگزار شد. داده‌ها با استفاده از تحلیل موضوعی بازتابی تحلیل شدند. دو موضوع شناسایی شد: میل به آگاهی و شناخته شدن و نبودن در دستان بسیاری. این موارد نشان‌دهنده میل زنان برای احساس امنیت و باور آن‌ها به این است که یک رابطه اعتماد با یک مامای شناخته‌شده در طول بارداری، زایمان و دوره پس از زایمان می‌تواند ترس آن‌ها را کاهش دهد. مراقبت استاندارد پراکنده به عنوان استرس‌زا درک شد و تمام زنان علاقه‌مند به مراقبت مامایی پیوسته ابراز داشتند. زنان دارای ترس از زایمان معتقدند که پیوستگی رابطه‌ای با یک مامای شناخته‌شده می‌تواند اعتماد، راهنمایی و ترس را کاهش دهد. مراقبت استاندارد پراکنده به عنوان نامطمئن درک شد که به آشوب عاطفی کمک می‌کند. این یافته‌ها نیاز به مراقبت مامایی شخصی‌سازی‌شده و رابطه‌ای را برجسته می‌سازد.

این پژوهش با طراحی کیفی انجام شد و مصاحبه‌های نیمه‌ساختاریافته تلفنی در دوران بارداری و پس از زایمان با زنانی که نمره‌شان در مقیاس ترس از زایمان بالای ۷۰ بود، برگزار شد. داده‌ها با استفاده از تحلیل موضوعی بازتابی تحلیل شدند.
