چکیده اصلی

PURPOSE: In recent years, minimally invasive procedures for benign prostatic hyperplasia (BPH) and urethral strictures have attracted great attention. There is an urgent need for studies investigating the safety and efficacy of the drug-coated balloon (DCB) dilation system (Keeway) for BPH. METHODS: The balloon was coated with paclitaxel (PTX). 21 male Labrador dogs were lastly randomized into the single-dose group (n = 12), double-dose group (n = 3), sham procedure group (n = 3), and blank control group (n = 3). The study period was 90 days. Blood and urine samples were collected before the procedure, and blood, urine, target tissues (prostate and bladder neck), and major organs (heart, liver, spleen, lung, kidney, and brain) were harvested at follow-up time points for subsequent processing and analysis. RESULTS: No significant PTX accumulation was observed in any specimen, and no pathological changes were detected in major organs, suggesting the safety of this approach. At 90 days, dilatation of the prostatic urethra was evident in both treatment and sham groups. CONCLUSION: This study suggested the safety and efficacy of the DCB dilation system (Keeway), encouraging further clinical investigation as a potential minimally invasive therapy for BPH, especially in Chinese population.

هدف: در سال‌های اخیر، روش‌های کم‌تهاجمی برای هیپرپلازی پروستات خوش‌خیم (BPH) و تنگی مجاری ادراری توجه زیادی را به خود جلب کرده است. نیاز فوری به مطالعات بررسی‌کننده امنیت و اثربخشی سیستم گشادکننده بالون داروپوشانده شده (DCB) (Keeway) برای BPH وجود دارد. روش‌ها: بالون با پaclitaxel (PTX) پوشانده شده بود. ۲۱ سگ لابرادور نر بالغ در نهایت به گروه دوز تکی (n = 12)، گروه دوز دوگانه (n = 3)، گروه عملیات جعلی (n = 3) و گروه کنترل خالی (n = 3) تصادفی شدند. دوره مطالعه ۹۰ روز بود. نمونه‌های خون و ادرار قبل از عملیات جمع‌آوری شدند و خون، ادرار، بافت‌های هدف (پروستات و گردن مثانه) و اعضای اصلی (قلب، کبد، طحال، ریه، کلیه و مغز) در نقاط پیگیری برای پردازش و تحلیل بعدی برداشت شدند. نتایج: هیچ تجمع معناداری از PTX در هیچ نمونه‌ای مشاهده نشد و تغییرات بیماری‌زایی در اعضای اصلی تشخیص داده نشد که نشان‌دهنده امنیت این روش است. در ۹۰ روز، گشاد شدن مجاری ادراری پروستات در هر دو گروه درمانی و جعلی مشهود بود. نتیجه‌گیری: این مطالعه اثربخشی و امنیت سیستم گشادکننده بالون DCB (Keeway) را پیشنهاد کرد و بررسی‌های بالینی بیشتر را به عنوان یک درمان کم‌تهاجمی بالقوه برای BPH، به ویژه در جمعیت چینی، تشویق می‌کند.

روش پژوهش
