چکیده اصلی

This case report describes a man in his 50 s who developed septic cavernous sinus thrombosis secondary to odontogenic maxillary sinusitis complicated by a non-aneurysmal subarachnoid haemorrhage (SAH). He presented with visual loss, altered consciousness and shock. CT revealed SAH near the left Sylvian fissure; right maxillary sinusitis and thrombosis of the cavernous sinuses, superior ophthalmic veins and internal jugular veins without an aneurysm. Despite intensive treatment, including broad-spectrum antibiotics, anticoagulants, tooth extraction and endoscopic sinus drainage, he developed a haemorrhagic venous infarction and subsequently died from cerebral herniation due to progressive brain swelling. Streptococcus dysgalactiae subsp. equisimilis (Group C Streptococcus) was isolated from both blood cultures and purulent material was aspirated during tooth extraction. This case highlights the pathogenic potential of Group C Streptococcus and the importance of recognising septic cerebral venous thrombosis as a cause of SAH.

موردی از مردی در دهه پنجاه زندگی که دچار خونریزی زیرپرداختی و ترومبوز سیاهان سیفونی عفونی ناشی از سینوسیت ماگزیلاری اودونتوجنیک شد. عامل عفونی استرپتوکوکوس دیسگالاکتیا سابسپ. اکوئیسیمیلیس بود. با وجود درمان‌های شدید، بیمار درگذشت.

  • خونریزی زیرپرداختی (SAH) می‌تواند ناشی از ترومبوز سیاهان سیفونی عفونی باشد.
  • عفونت اودونتوجنیک می‌تواند منجر به ترومبوز سیاهان سیفونی و خونریزی شود.

این گزارش موردی مردی در دهه پنجاه زندگی را شرح می‌دهد که دچار ترومبوز سیاهان سیفونی عفونی ثانویه به سینوس ماگزیلاری اودونتوجنیک و خونریزی زیرپرداختی غیرآنوریسمال (SAH) شد. او با از دست دادن بینایی، تغییر هوشیاری و شوک ظاهر شد. سی‌تی نشان‌دهنده خونریزی زیرپرداختی نزدیک شیار سیلویان چپ، سینوسیت ماگزیلاری راست و ترومبوز سیاهان سیفونی، سیاهان اپتیک فوقانی و سیاهان سیفونی داخلی بدون آنوریسم بود. با وجود درمان‌های شدید شامل آنتی‌بیوتیک‌های طیف گسترده، آنتی‌کوآگولانت‌ها، کشیدن دندان و تخلیه سینوس‌ها از طریق لوله، او دچار سکته وریدی خونی شد و در نهایت بر اثر سندرم غشای مغزی ناشی از تورم پیشرونده مغز درگذشت. استرپتوکوکوس دیسگالاکتیا سابسپ. اکوئیسیمیلیس (گروه C استرپتوکوک) از هر دو کشت خون و ماده پورولنت در حین کشیدن دندان جدا شد. این مورد اهمیت شناسایی ترومبوز سیاهان سیفونی عفونی به عنوان علت SAH و پتانسیل بیماری‌زایی گروه C استرپتوکوک را برجسته می‌کند.
