چکیده اصلی

OBJECTIVE: This study aimed to unveil the right atrial function in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) using two-dimensional speckle tracking echocardiography (2D-STE). METHODS: A total of 120 OSAHS patients were enrolled as the OSAHS group, with 100 healthy individuals served as the control group. Conventional echocardiography assessed right atrial structure, including superior-inferior diameter, transverse diameter, area, and volume. Myocardial longitudinal strain parameters (reservoir strain (RASr), conduit strain (RAScd), and contraction strain (RASct)) were measured using 2D-STE. Comparisons were made between groups, and patients were further stratified by apnea-hypopnea index (AHI) into mild (5-14 events/h), moderate (15-39 events/h), and severe (≥ 40 events/h) subgroups. Correlation with AHI was assessed by Pearson correlation analysis. RESULTS: Compared to controls, the OSAHS group showed progressive decreases in RASr and RAScd, and increases in RASct and transverse diameter with increasing disease severity (p < 0.05). AHI was negatively correlated with RASr and RAScd, and positively correlated with RASct and transverse diameter (p < 0.05). CONCLUSION: Patients with OSAHS exhibit significant structural and functional alterations of the right atrium. 2D-STE provides a reliable, non-invasive method for early assessment and clinical evaluation of right atrial dysfunction in OSAHS patients.

هدف این مطالعه، بررسی عملکرد اتاق راست در بیماران با سندرم آپنه و هیپوپنه خواب مختل (OSAHS) با استفاده از اکوکاردیوگرافی ردیابی اسپیکل دو بعدی (2D-STE) بود. در این مطالعه، ۱۲۰ بیمار OSAHS و ۱۰۰ فرد سالم به عنوان گروه کنترل وارد شدند. اکوکاردیوگرافی معمولی برای ارزیابی ساختار اتاق راست شامل شعاع طولی، قطر عرضی، مساحت و حجم استفاده شد. پارامترهای کژش عضلانی قلب (کژش مخزن RASr، کژش مجاری RAScd و کژش انقباضی RASct) با استفاده از 2D-STE اندازه‌گیری شدند. بیماران بر اساس شاخص آپنه-هیپوپنه (AHI) به سه زیرگروه خفیف (۵-۱۴ رویداد/ساعت)، متوسط (۱۵-۳۹ رویداد/ساعت) و شدید (≥ ۴۰ رویداد/ساعت) طبقه‌بندی شدند. همبستگی با AHI با تحلیل همبستگی پیرسون ارزیابی شد. نتایج نشان داد که در مقایسه با کنترل‌ها، در گروه OSAHS، با افزایش شدت بیماری، کاهش تدریجی RASr و RAScd و افزایش RASct و قطر عرضی مشاهده شد (p < 0.05). AHI به صورت منفی با RASr و RAScd و به صورت مثبت با RASct و قطر عرضی همبستگی داشت (p < 0.05). نتیجه‌گیری: بیماران مبتلا به OSAHS تغییرات ساختاری و عملکردی قابل توجهی در اتاق راست نشان می‌دهند. 2D-STE یک روش مطمئن و غیرتهاجمی برای ارزیابی زودهنگام و بالینی عملکرد ضعیف اتاق راست در بیماران OSAHS ارائه می‌دهد.

روش پژوهش

این مطالعه یک مطالعه مقایسه‌ای با گروه کنترل بود. ۱۲۰ بیمار OSAHS و ۱۰۰ فرد سالم وارد شدند. عملکرد اتاق راست با 2D-STE و اکوکاردیوگرافی معمولی ارزیابی شد.
