چکیده اصلی

CASE: A 73-year-old woman with a custom triflange acetabular component (CTAC) underwent re-revision total hip arthroplasty (THA) with prophylactic contralateral vascular access. Severe acetabular bone loss led to medialization of the CTAC, putting the distal external iliac artery at risk during surgery. Endovascular access allowed for immediate control of hemorrhage if encountered. Patient was symptom-free at 1 year. CONCLUSION: This case demonstrates the feasibility and safety associated with obtaining pre-emptive vascular access for high-risk revision THAs. This methodology allows for potentially decreasing patients' morbidity and mortality, without incurring significant additional risks.

در این گزارش، یک زن ۷۳ ساله تحت بازسازی مجدد کل پروتز مفصل ران با استفاده از مولفه آکتئیبولوم مخصوص سه لایه (CTAC) قرار گرفت. به دلیل از دست دادن شدید استخوان آکتئیبولوم، مولفه به سمت میانی منتقل شد و ریسک آسیب به شریان بیرونی ایلیاک تحتانی را در حین جراحی ایجاد کرد. دسترسی عروقی در سمت مخالف به عنوان یک اقدام پیشگیرانه انجام شد تا خونریزی در صورت بروز کنترل شود. بیمار در یک سال پیگیری بدون علامت بود.

یک زن ۷۳ ساله با استفاده از یک مولفه آکتئیبولوم مخصوص سه لایه (CTAC) تحت بازسازی مجدد کل پروتز مفصل ران (THA) با دسترسی عروقی پیشگیرانه در سمت مخالف قرار گرفت. از دسترسی عروقی برای کنترل فوری خونریزی در صورت بروز استفاده شد. بیمار بدون علامت در یک سال پیگیری بود. این مورد نشان‌دهنده امکان‌سنجی و ایمنی دسترسی پیشگیرانه عروقی در بازسازی‌های مجدد پرخطر THA است. این روش می‌تواند منجر به کاهش عوارض و مرگ‌ومیر بیماران بدون ایجاد ریسک‌های اضافی قابل توجه شود.
