چکیده اصلی

CASE: A 14-year-old boy developed chronic first-metatarsal osteomyelitis and was left with a 4-cm defect after thorough debridement. Initial management included radical excision, insertion of a gentamicin polymethyl-methacrylate spacer supplemented with vancomycin and application of a mini external fixator to preserve length and alignment. After normalization of inflammatory markers, the second stage used induced-membrane preservation with reconstruction using a nonvascularized fibular strut graft, cancellous iliac crest graft, and low-profile plate fixation, achieving union and full functional recovery. CONCLUSION: A modified Masquelet approach with temporary external fixation and structural fibular grafting offers a viable limb-salvage solution for pediatric first-metatarsal osteomyelitis.

در این گزارش موردی، یک نوجوان ۱۴ ساله با عفونت مزمن استخوان مچ پا درمان شد. پس از جراحی‌های متعدد و استفاده از اسپیسر آنتی‌بیوتیکی و تثبیت خارجی، بافت نرم ایجاد شد و با graft‌های استخوانی و تثبیت‌کننده‌های مخصوص، اتصال استخوانی و بازسازی موفقیت‌آمیز حاصل گردید.

  • عفونت مزمن استخوان مچ پا در نوجوانان می‌تواند با تکنیک‌های جراحی پیچیده مدیریت شود.

یک پسر ۱۴ ساله دچار عفونت مزمن استخوان مچ پا شد و پس از پاکسازی کامل باقی‌مانده ۴ سانتی‌متری باقی ماند. مدیریت اولیه شامل برداشتن رادیکال، تزریق اسپیسر گنتامایسین پلی‌متیل‌متاکریلات همراه با وانکومایسین و استفاده از یک تثبیت‌کننده خارجی کوچک برای حفظ طول و تراز بود. پس از نرمال شدن نشانه‌های التهابی، مرحله دوم شامل حفظ حفره‌ای که با بافت نرم ایجاد شده بود و بازسازی با graft استخوان فیبرولا (غیرعروقی)، graft استخوان توده ایلئک و تثبیت با صفحه کم‌پروفیل بود که اتصال و بهبود کامل عملکردی را به دست آورد. نتیجه‌گیری: رویکرد ماسکولت اصلاح‌شده با تثبیت خارجی موقت و graft استخوان فیبرولا، راهکاری قابل اتکا برای نجات اندام در عفونت استخوان‌های مچ پا در کودکان است.
