چکیده اصلی

OBJECTIVE: The purpose of this study was to compare the alveolar ridge preservation (ARP) effect using reactive soft tissue (RST) for socket sealing to guided bone regeneration (GBR) with collagen membrane and sponge in type IV molar sockets. MATERIALS AND METHODS: This retrospective study included 40 sites. Bone regeneration outcomes were evaluated by comparing CBCT images preoperatively (T0), immediately postoperatively (T1), and at a 6-month follow-up (T2). Measurements included changes in horizontal ridge width, vertical ridge height, graft material retention rate, post-operative surgical complications, incidence of secondary bone grafting at implant placement, and short-term implant survival rate. RESULTS: Both groups showed a significant increase in horizontal bone width at 1 mm, 3 mm, and 5 mm below the crest, with no differences. From T2 to T1, the RST group demonstrated superior vertical stability at the crest center (gain of 0.34 mm vs. loss of 0.88 mm in controls, P = 0.023) and a higher graft material retention rate (92.84% vs. 68.04%, P = 0.003). No significant difference was found in the secondary bone grafting rate, and the short-term implant survival rate was 100% in both groups. CONCLUSIONS: RST combined with DBBM reduced ridge resorption in severe defect molar sites. It shows a better tendency in preserving vertical ridge height and graft retention compared to the GBR. CLINICAL RELEVANCE: Using RST for socket sealing achieves comparable bone preservation to the GBR in severe defect molars, while simplifying the surgical procedure and reducing costs.

این مطالعه بازگشت‌نگر اثر بافت نرم واکنشی (RST) را در مقایسه با رگولاسیون استخوان هدایت‌شده (GBR) در سایت‌های مولر با نقص شدید بررسی کرد. نتایج نشان داد که RST در حفظ ارتفاع لبه عمودی و نرخ حفظ ماده graft بهتر عمل می‌کند. نرخ بقا ایمپلنت در هر دو گروه ۱۰۰٪ بود.

هدف این مطالعه مقایسه اثر حفظ لبه آلوئولار (ARP) با استفاده بافت نرم واکنشی (RST) برای بستن لوله دندان و رگولاسیون استخوان هدایت‌شده (GBR) با غشای کلاژن و اسفنج در لوله‌های دندان مولر از نوع IV بود. این مطالعه بازگشت‌نگر ۴۰ سایت را شامل شد. نتایج رگولاسیون استخوان با مقایسه تصاویر CBCT پیش از عمل (T0)، بلافاصله پس از عمل (T1) و پیگیری ۶ ماهه (T2) ارزیابی شد. اندازه‌گیری‌ها شامل تغییرات عرض لبه افقی، ارتفاع لبه عمودی، نرخ حفظ ماده graft، عوارض جراحی پس از عمل، شیوع grafting استخوان ثانویه در قرارگیری ایمپلنت و نرخ بقا کوتاه‌مدت ایمپلنت بود. هر دو گروه افزایش معنی‌دار عرض استخوان افقی در ۱، ۳ و ۵ میلی‌متر زیر قله را نشان دادند که تفاوتی نداشتند. از T2 به T1، گروه RST پایداری عمودی برتر را در مرکز قله نشان داد (افزایش ۰.۳۴ میلی‌متر در مقابل کاهش ۰.۸۸ میلی‌متر در کنترل‌ها، P = 0.023) و نرخ حفظ ماده graft بالاتری داشت (۹۲.۸۴٪ در مقابل ۶۸.۰۴٪، P = 0.003). نرخ grafting استخوان ثانویه تفاوت معنی‌داری نداشت و نرخ بقا کوتاه‌مدت ایمپلنت در هر دو گروه ۱۰۰٪ بود. نتیجه‌گیری: RST ترکیبی با DBBM در سایت‌های مولر با نقص شدید، رگولاسیون لبه را کاهش داد. این روش تمایل بهتری در حفظ ارتفاع لبه عمودی و حفظ graft نسبت به GBR نشان می‌دهد.

روش پژوهش
